I have a flush in.my face and ears that recently started. do not smoke, no drink, fairly healthy, was wondering if this could be a sign of a pheochromocytoma or carcinoid tumor since my chromogranin test was negative and all.my hormones are in normal range. the only med I take is flexeril for migraine but I have never have this before.
Brief Answer:
Unlikely to be Pheochromocytoma
Detailed Answer:
Thank you. Saw the reports. Pheochromocytoma cause episodic palpitations, headaches, high blood pressure, flushings and sweating. However, most commonest cause of these symptoms is anxiety/panic episodes rather than Pheo.
Isolated Facial flushing is unlikely to be due to Pheo unless accompanied by other episodic symptoms which I mentioned earlier. Since Chromogranin-A levels are normal, Carcinoid is unlikely. I hope you had made diet modifications prior to the chromogranin-A test.
One important point to note is that, Flexiril can cause photosensitivity reactions similar to what you have explained.
All other hormone levels are very much in normal range.

I started my diet my modification after the chomogranin and hormone levels, why? you mentiod before. should I get tested them for pheochromocytoma? why would the flexeril be causing if it is a flushing now after months taking it? don't you think it would have started since the first time I took it. Another point last night I had an episode of flushing and instead of my face flushing first one ear lobe got really red and the both at the same time, my face was cool. not hot at all.
Brief Answer:
Diet modification before Chromogranin-A
Detailed Answer:
Before doing Chromogranin-A, certain dietary restrictions are needed. I hope your doctor/dietician had advised the same.
I just suggested that Flushing/photosensitivity is a side effect of Flexeril. If you have episodic palpitations, headaches , you may request for a pheochromocytoma work up.
Flushing can be occasionally seen due to allergic reactions to drugs/food etc.

no my doctor did not suggest the diet restrictions before the chromogranin but anyways the chromigranin Is within normal limits right? so you think I need to be concern about pheochromocytoma?
Brief Answer:
need physicall examination
Detailed Answer:
I do not suspect pheochromocytoma just based on the symptoms which you provided. However, there are limitations for online consultations. You may need a thorough physical examination before any test.
As I mentioned earlier, if you have palpitations, episodic headache or sweating, pheochromocytoma should be suspected. It is a rare disease and doctors often overlook it.

what can I tell the doctor so he won't overlook it and get me tested for phoechromocytoma do you think carcinoid is ruled out
Brief Answer:
tests
Detailed Answer:
You should explain the symptoms. If you have episodic symptoms, then pheo is worth excluding.
Explain about negative chromogranin test too.
We'll a negative chromogranin may not fully rule out carcinoid. Other tests may be required. One other test is 24 hour urine 5HIAA test

Brief Answer:
Guided and focused investigations
Detailed Answer:
In that case, you may request your PCP to refer you to an endcrinologist. An endocrine evaluation is essential and that would throw some light in to your situation.
Instead of doing so many tests, it is always better to talk to patient in detail and come to a conclusion and then do a focused investigation rather than doing investigations blindly.
I hope you will be guided well by your endocriologist. Wish you a fast recovery.

I went to my primary doctor today and blood pressure was 149/96 its kind of been going up and down. he order a bi lateral ultrasound of my kidneys to make sure there's no problem. I don't feel any pain and my urine is clear. he told.me that pheochromocyta has other symptoms like you mentioned and has a much more higher blood pressure than mine. He also.so prescribed atenolol 50mg once a day for blood pressure. How long does it take for blood pressure to lower with this medication, and what are your thoughts of something bad wrong with my kidneys?
Brief Answer:
Choice of BP pill
Detailed Answer:
Good morning,
I do agree with your doc. However, i would disagree with your current choice of Blood pressure medicines. I also do not agree that, you need to have a much high BP to diagnose Pheo. At your age ( 31 year), we need to investigate the cause of high blood pressure. Doing an ultrasound is a good idea. We need to look at kidneys and also do a Doppler study of kidney blood vessels.
When we evaluate a young hypertensive patient, we do screen for Pheo too. By the way do you have any neck swellings ( thyroid swellings?).
While we have not fully investigated ( or not rule out Pheo completely), Atenolol is not a great option in your case. Because, it can worsen the pheochromocytoma ( if you have one). Besides, in a young person like you, i would not start Atenolol.
I would recommend Prazocin 2.5 ( Minipress) at night to start with. We can titrate the dose later. This is the safest drug whilst the cause of your high blood pressure is being evaluated.
PLEASE TRY TO SEE AN ENDOCRINOLOGIST.. Gently request your primary care Doc..
Meanwhile, please relax. Anxiety itself can push your BP up.
