I have gone through all your medical reports.CT cranium and ECG are normal.ECHO reveals mild aortic sclerosis which is age related.Mild LVH and mild AR are reported which are unlikely to result in the present symptoms. These changes are commonly associated with Hypertension.Do you have any history of high blood pressure?
To summarise, nothing significant in these reports which can result in such symptoms.

Brief Answer:
Neurological examination/Hypothyroidism management.
Detailed Answer:
Hi XXXXXXX
Thanks for being in follow-up.
I have gone through further reports. He is having mildly low sodium levels(hyponatremia) and low T3 levels(mild).Though, the thyroid picture otherwise is that of a Non-thyroidal illness as TSH levels are normal. However,in setting of existing Hypothyroidism,I suggest that Thyroid hormones intake dose should be increased a bit and repeat Thyroid function tests should be done after 6-8 weeks.Hypothyroidism can be a potential cause of hyponatremia. It has to be looked whether Thyroid hormones correction improves Hyponatremia or not.For now,I suggest you to get a repeat serum Electrolytes done from a standard lab.
Further,Endocrinologist's opinion can be sought over the abnormal Thyroid function tests.
Weakness in walking and standing from the sitting position,in the background of Diabetes and Hypothyroidism mandates a neurological examination to rule out neuropathy or myopathy. Please consult the a neurologist.
If you have any further questions,I shall be glad to answer.
