
What Do These ESR And HDL Levels Indicate?

Question: As per recent pathology tests done yesterday, i have ESR value is 24 [where as normal value is14] and HDL value is 234[where as normal value is 200], could you please advise the reason for the same along with correct line of treatment. Can these values be ignored or there are some concerns with the values, i am currently 45yr with 80kgs weight and 170cm height.

1. Please provide your complete lipid profile including the values of Total, LDL and HDL cholesterol (including the units used by the lab)
2. Please provide your usual blood pressure in numbers.

1. The ESR value is 24mm at first hour which is marginally above the high normal value of 14mm of the lab. Though high ESR usually indicates a disease process, marginally high ESR in absence of any symptoms of disease, as stated by you, may be a normal variant. The normal value of TLC and DLC also do not favor infection or inflammation. Various diseases in which it may be high are infections inflammation, cancer, kidney failure, anemia, autoimmune diseases etc. It may be slightly higher in old age, pregnancy and getting a test done after having meals. since there are no symptoms and levels are marginally high, best way is to get them repeated after sometime like 2-3 weeks. Do watch for development of any symptoms.
2. Your total cholesterol value is 234 and not HDL value. The HDL cholesterol is 41mg% and LDL cholesterol is 163mg%. Moreover, the value of blood pressure, i.e. 150mmHg as stated by you, is higher than normal limit of 140mmHg. Your BMI is 26.9. All these factors needs attention. Your body weight should be around 67Kg. Hence you need to reduce your weight. Since blood pressure is higher than normal it requires regular monitoring. If multiple readings are more than 140/90mmHg, you will require to start medication for same. However, for the cholesterol level with this profile, medication is not warranted as yet. Only life style modification followed by a repeat profile after 3 months is warranted. Try to devote more time to exercise with a goal of 45 minutes of brisk walk per day or equivalent exercise. Does eating from 4 chapaties reducing to 3 can make difference in weight management? Also let me know when to drink water when having meals like in between or at last?Does that make diff. if i am eating regularly 2 light meals and 2 main meals every 3-4hours ?
