What Do These Echo Images Indicate?

Question: Dr.

I got my echo report whereas it shows 1.2 cm and the images shows 1.27 cm. Actually it is not 1.1 cm as previously told. I asked Dr. again today he was upset and he told me that I have made mistake of explaining this to you. you are okay just keep the BP under control, visit me every month. You pathology of heart is very good. So need to worry. you need to worry when it reaches more than 1.5 cm. I need assessment of the report as well as the images for your review. How long shall it take to bring back the LVH to normal (<1.1 cmm).

I passed very carefully through your Echo imagines and reports. I am glad to inform you that almost every measured parameter is normal (cavity dimensions, both left and right, diastolic and systolic ventricular functions, valvular morphology and function, pericardial and large vessels also, etc).

Facing the above results, I would add that the mild interventricular septum hypertrophy has no implications at all on cardiac function (as there is no evidence of any increased left ventricular systolic outflow gradient or any abnormally altered LV inflow pattern).

Your doctor is right, when he says that your main goal is to correctly control your high blood pressure.

The meticulous use of the appropriate antihypertensive therapy will consequently stop the progression of mild septal hypertrophy to a more excessive pattern, and in a the most favorable scenario (if blood pressure values are optimally controlled), it may reverse mild hypertrophy to a normal thickness within 1 year.

You should be happy even to stop progressing the mild septal hypertrophy, as it actually doesn't exert any adverse implications to your heart.

You need to perform regular follow ups to your attending doctor.

What about the physical activities and other measures dr.? Since one week Most of the time my BP is between 130/75. At tumes it goes 140/80 and come back to normal, this is with medication. Cariologist tole me ladt week 1.1 cm, today when I saw the report, it was 2.0 cm and the image show 2.7 cm. very strange. When I met cardio today, the BP was 110/70, afer two hours of medication.

(a) You are recommended to follow a physically active life style. That is, at least one hour per day of physical activity (fast walking, cycling, swimming, running, playing football, or aerobics, etc. A healthy diet is also encouraged (I strongly recommend you to follow Mediterranean diet).

(b) Regarding your optimal BP values, it is recommended to not exceed 125/75 mm Hg, optimally around 110/70 mmHg (to expect a beneficial possible effect of antihypertensive therapy on hypertrophy reversal).

(c) Don't worry about those differing values: 1.1 cm, 1.2 cm, or even 1.27 cm. As I have explained you before, these values (as the echo report concludes), are not involved in any cardiac function alterations at all.

Cardiac ultrasound is well known for some subjective aspects, when dealing with dimensions (inter-observer and even intra-obsever variability regarding measured values of the same parameter).

Dr. I am afraid that controlling BP to 125/75 would be difficult, i am sure i can keep siastolic to 75 but systolic would be difficult as most of the time i spent in the office. At home i think i can control it.

It is very important to keep your BP values under close control all the day long.

If you really want to not just stop the progression of that mild myocardial hypertrophy, but even to expect a hypertrophy reversal, you should always keep an eye opened on your BP values.

First you need to closely monitor your BP as frequently as you can for a couple of days. If BP values greater than 125/75 mmHg are persistent, then probably you would need to increase a little bit your daily antihypertensive drug dose.

Just talk to your attending doctor about that issue.
