Question: Hello Dr XXXXXXX

This is in continuation with my last query regarding my father's disease - Cancer Of the prostate (http://www.healthcaremagic.com/AskDoctorInboxServlet?page=viewQuery&queryId=262828).

Currently, it has been diagnosed through MRI that the Cancer has slightly moved to adjacent area of the Prostate and thus removal of Prostate is not going to remove complete cancer. Since, my father has low Uroflow so he's been asked to go for Hernia Surgery first along with urinary passage correction.
Post recovery, Radiation Therapy and Hormonal Therapy will be given to cure Cancer.

I request you to please check MRI and Bone Scan reports attached and share your thoughts.

Brief Answer:
Your father has Locally spread cancer of the prostate .

Detailed Answer:
Hello
Thanks for your query ,based on the facts and report of MRI and Bone Scan of your father that you have posted it appears that he has Cancer of the Prostate which has infiltrated beyond the capsule of the prostate but there is no distant metastasis (in the bone)

In a given situation your Urologist has rightly advised to go for resection of Prostate to make urinary passage free from obstruction with surgery for Hernia repair .to be followed by radiotherapy and hormonal treatment to control the further spread of the disease .

Since cancer of the Prostate is Testosterone dependent primary aim of the treatment is to cut off the production of Testosterone by doing Bilateral Orchidectomy (Removal of Testicles ) supported by hormones to arrest the further growth of the cancer to some extent .(Your Urologist might have explained you the rationale of the treatment and outcome of the treatment )

Once the cancer has spread beyond capsule and infiltrated in surrounding tissue it is imperative to monitor it by doing serum PSA at frequent intervals so as to modify the treatment and to make life of the patient pain free .

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
