Brief Answer:
He should undergo angiography

Detailed Answer:
Hello,
I've reviewed the reports,

Firstly, it's important to know what kind of chest pain is it, does it occurs at rest or occurs on exertion. The various possibilities in his case are, Cardiac pain, pain due to gastritis and reflux disease and if he smokes then it can also cause pain.

Cardiac pain is the first possibility as TMT is positive, but sometimes can be falsely positive. Cardiac pain becomes further likely if this pain is occurring on excretion and relieves with rest.

If it is occurring at rest and associated with other symptoms like abdominal pain, nausea, burning in chest or increase in the pain after food then it raises the possibility of gastritis and reflux disease.

So overall, considering his TMT result, which is positive which suggests that there are blockages in his heart. So he should undergo angiography to confirm and know the extent of blockages. His echo and ECG are normal. So he should also be started on Tab Ecosprin AV 75/20 mg once a day bedtime and Tab Sorbitrate 5 mg sublingually whenever Chest pain occurs, till we confirm the diagnosis.

Also, get his serum lipid profile and sugars checked. Have a healthy lifestyle like low calorie, low-fat diet, and daily walking.

Hope this helps you and get back if you have any doubts or more information to share.
Regards
