Brief Answer:
Cerebrovascular accident (CVA)

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
First of all please attach the reports.
Based on the history I would mention here that her uncontrolled diabetes is the cause for CVA commonly known as Stroke.
She would requite hospitalization along with anti platelet drugs like Aspirin and Atorvastatin for her infarcts.
She would also require strict control of Diabetes initially with insulin any later with drugs if feasible.
Her syncope could be related to uncontrolled long standing Diabetes and a carotid Doppler should be performed to rule out any blockages in carotid arteries.
I hope it helps you.
If you have any further query, please ask me.
