What Do These Fibroscan, ECG And Treadmill Test Reports Indicate?

Question: Dear Sir,
I am an XXXXXXX Male presently residing in Abu Dhabi and have following parameters.
Age: 35 Yrs
Weight: 93 Kgs
Height: 5 ft 6 in
BMI: 33

PRESENT CONDITIONS
(1) Palpitations / Hypertension
(2) Alcoholic Liver Disease

PAST HISTORY
(1) Kidney stone and pain due to OTC medicine for Hydrocele, 13 Years ago
(2) Hepatitis due to contaminated Water in College Hostel, 10 Years ago

PRESENT MEDICATIONS for Heart
(1) CONCOR COR 2.5 MG - Once Daily in Morning (Started taking from 5th July 2016)
(2) CRESTOR 5 mg - Once Daily in Night (Started Taking from 2 Weeks)
(3) ASPIRIN 75MG (enteric) - Once Every Alternate Night with Crestor (Started Taking from 2 Weeks)

PRESENT MEDICATIONS for Liver
(4) URSODIOL 250 MG - One Tab Twice Daily, Morning and evening, after food (Started taking from 5th July 2016)
(5) MILK THISTLE - One Tab Twice Daily with ursodiol (Started Taking from 1 week)
(4) VITAMIN E - One Capsule Once Daily in Morning (Started taking from 5th July 2016)
(5) Ayurvedic Medicine like Arguna Capsule and Hridayamruth Once Daily in Afternoon

REPORTS ATTACHED
(1) Treadmill Test
(2) ECG
(3) Blood Test
(4) Fibroscan
(5) Echo Cardiogram

MY SPECIFIC QUESTIONS
(1) What is your expert opinion on FIBROSCAN and LFT
(2) What is the outlook/advice on Current Medications for Liver?
(3) What is your advice on the above mentioned prescribed/non-prescribed medications, do i have to change anything or continue with the same?
(4) Why sometimes my liver area use to ache after taking Crestor 5mg?
(7) Kindly advice alternatives if possible as i am only 35 yrs old and i think i am young for these medicines with no family history of Heart disease except Cholesterol and slight BP for both my parents

Hello,
I apologize for late reply
1) Fibroscan shows mild damage to liver. This may be due to your habit of alcohol consumption. Damage is so mild that liver function test is normal and this damage is detected only by high-end test like Fibroscan. I recommend to you to undergo USG with Doppler of abdomen specific for liver and portal flow.
2) Reason for USG - a) it will give idea about any structural abnormality in liver which can happen due to alcohol (i.e. early stage of cirrhosis, fatty liver), Your platelets are high and you are on blood thinning medication so to rule out thrombus (blockage in blood vessel due to accumulation of platelets) formation in portal vein which supply 80% blood to liver, Doppler study of liver is necessary.
3) Your cholesterol and triglyceride levels are high which may give rise to condition - fatty liver in which fat deposit occur in liver tissue. Early stage of this disease is reversible but moderate to severe fatty liver is irreversible. Alcohol + fatty liver is high risk for development of cirrhosis
4) STOP ayurvedic medicines, strictly because these medicines damage liver
5) Pain on liver area may be due to slight enlargement of liver from fatty deposition in liver tissue or mid dysfunctioning of liver function (even so mild that not detected in liver function test)
6) Advices- a) Strictly STOP ayurvedic medicines, b) regular exercise, c) reduce fat containing diet, d) high-protein diet, e) stop alcohol drinking, f) get USG with Doppler study of liver and portal system
7) About your medications - I think Tablet Ursodiol and Vitamin E is not necessary (As no evidence of liver damage, may be you should re-consider these medications based on USG report), Stop ayurvedic medicines. Drink cleaner water.

Dear Sir,

Many Thanks for your kind reply.

I am attaching my USG and X-ray report of abdomen.

Kindly advice your expert opinion on the same in coordination with Fibroscan report.

Also Advice the following:

(1) I have symptoms of IBS and for the same, i am taking NEXIUM 40mg once in morning empty stomach with DUSTAPTALIN one capsule twice daily in AM and PM empty Stomach. Do these drug interact with URSODIOL?

(2) I am also taking CRESTOR 5mg once daily at night. Do this drug have a bad impact on Liver or does it interact with URSODIOL?

(3) Is the combination of URSODIOL with MILK THISTLE, is OK or do i have to remove one.

(4) Kindly advice your expert opinion on any other medicine to be used for my liver instead of URSODIOL

Hello,
I apologize for late reply
1) If your present bilirubin level is normal and you had taken Ursodiol for 3 months then you should discontinue it after consulting your physician
2) You can continue Milk Thistle, Nexium along with Ursodiol.
3) About your USG report, It shows grade-1 fatty liver (fat deposition in liver tissue from higher cholesterol level + alcohol consumption). This is mild form which did not affect functionality of liver and this is reversible form i.e. with life-style change, diet-habit you can revert it.
4) Take care for fatty liver - a) avoid junk food, b) no oily food, c) high protein diet, d) intermittent fasting (1-2 per week), e) regular exercise, f) use of omega-3 fatty acid containing oil for food preparation, g) stop alcohol consumption
5) Fatty liver + alcohol liver disease carry slight higher risk of progression than only fatty liver. If it progress to severe fatty liver then it would be slowly irreversibly progress to cirrhosis of liver. Therefore this is the golden period from where you should start working on prevention.
