Detailed Answer:
Hello!

Welcome on - Ask a Doctor - service!

I reviewed your uploaded ECG and would like to explain that it shows an incomplete right bundle branch block pattern (iRBBB); that is, due to your chronic pulmonary disorder (cystic fibrosis) the right heart (that is responsible for the pulmonary blood circulation) may have undergone structural changes with an underlying electrical impulse delay (that is shown on your ECG).

But, from the other side, your medication (Nortriptyline) may lead to some degree of conduction delay with RBBB pattern (Brugada syndrome).

In addition, a fast heart rate is present, that could be due to some degree of respiratory dysfunction or to any potential medication adverse effect (Nortriptyline).

Coming to this point, I would recommend discussing with your attending doctor on the possibility of performing a pulmonary check up, including:

- respiratory function tests,
- an arterial blood gas analysis

From the other hand, it is necessary to discuss with your prescribing physician about a regular follow up of your therapy (including periodic ECGs, liver and renal function tests, blood electrolytes level).

At the end, I recommend performing a cardiac ultrasound for a better view of your structural cardiac conditions.

You should discuss with your doctor on the above mentioned issues.

Hope to have been helpful to you!

In case of any further questions, feel free to ask me again.
