
What Do These Following Blood And Urine Reports Indicate?

Hello Dr.,
From my blood test done 3 weeks ago my liver function test
was normal including bilirubine level (see p1).
The urine test included negative bilirubine but a track of urobillonegen. (see p2: spoor means "track").
What does this test tell?
I have fatty liver.
Please follow explanations below..

I read your query and understood your concerns.
You are reporting to have traces of urobilinogen in urine.
Urobilinogen is produced in the intestinal tract as a result of bacterial (intestinal bacteria) transformation of bilirubin. As the urobilinogen is absorbed from intestinal mucosa, it travel via blood to the liver and is secreted to billiary tract.
Only a small portion of it is eleminated threw the kidneys and appears as urinary urobilinogen. Up to 1 mg /dl of urobilinogen is presented in normal urine. If the result is up to 2.0 mg /dl than it is highl level and need to be evaluated what has caused it.
At your case you have track urobilinogen level; it means that is it difficult to measure because it is a small portion of urobilinogen at your urine. So, the levels of urinobilinogen are within normal values (les then 1 mg/dl) and there is nothing to be worry about.
Moreover the test results shows that you have only mild fatty liver disease and there's no liver damage at all. Otherwise there would have been a raised urobilinogen levels on your urine.

Still I have some questions:
1- My test does not give any number on the amount of urinobilinogen.
If the, the levels of urinobilinogen are within normal values (les then 1 mg/dl) according to your claim, then why is the result of test not "negative" but
said "track".
2-From my blood test result, how did you come to the conclusion that I have mild fatty liver? I do not think there is anything from blood test which suggests this.
3- What is the clinical meaning of negative bilirubine in irine but a track of urobillonegen? does this point to an advanced liver disease?

Let me explain again and try to clarify your thoughts.
1. When the levels of urobilinogen are lower than 0.1 mg/dl are printed (and considered) on urinalysis as a track of urinoblinogen. This is why I consider that your urinalysis was normal, because the urinobilinogen levels below 1 mg/dl are considered to be normal.
2. Based on all your medical data there's no evidence of liver cell damage and this is why I think that you have only mild liver fatty disease. However other data (especially abdominal ultrasound are needed for a better evaluation of the fatty liver disease).
3. No the contrary is true. You have a normal levels of bilirubin and a track of urinobilinogen in urinalysis (both within accepted levels) and these findings exclude the possibility that you have an advanced liver disease.
