What Do These Following Blood And Urine Reports Indicate?

default
Posted on Thu, 23 Feb 2017
Question: I just had blood/urine work at XXXXXXX Clinic. My MPA IgA was less than 7 and my MPA IgM was 25. My RNP antibody was 2.9. I was seeking a second opinion on an MSA diagnosis from Stanford and saw the specialist at XXXXXXX Clinic who stated I "didn't fit the profile for MSA" and ordered these tests. Stanford's rationale was a 43 point drop on tilt table worsening from 31 a year previous T. I also had a TST where I did not sweat over 90% of my body. The reason for testing was 2 episodes (1 4 minute and 1 36 hr) episodes of ataxia with no other symptoms and nothing showing on CT. I was unable to have an MRI at the time due to legacy pacemaker leads that were removed a couple of days ago at XXXXXXX My main question is what might these blood/urine numbers mean and what if any follow up should there be?

Brief Answer:
? Multiple system atrophy (MSA) with auto-immune disorder(MPA)

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
Multiple system atrophy (MSA) is a clinical diagnosis essentially.
The diagnosis of MSA however has been aided modestly by imaging techniques. Both MRI and CT scanning frequently show atrophy of the cerebellum and pons in those with cerebellar features (ataxic). Your CT scan brain is normal.Hence MRI brain may throw some light on this.

Microscopic polyangiitis an autoimmune disease with main diagnostic test being perinuclear antineutrophil cytoplasmic antibodies (p-ANCA) with myeloperoxidase specificity.Microscopic polyangiitis is strongly ANCA-associated.
Kindly clarify whether RNP is snRNPs (Small nuclear ribonucleoprotein particles ) or hnRNP (Heterogeneous nuclear RNP).
Kindly upload your serum report of MPA IgA and MPA IgM report with reference range mentioned for further analysis.
