What Do These Following Lab Reports Indicate?

Question: Hi Doctor
My Aunty Age is 59years, She is suffering from Jaundice from last 3 weeks. Initially doctor has given medince for jaundice. since there is not improvement doctor has next blood test, ultra sound scan. ultra sound scan showed some abstraction in pancreatic head. Then the doctor has suggested the CT scan and MR scan. I am attaching all the reports. Would you please tell us what is the situation, how critical the situaltion and what is the medication? Is any surgery required or it can be handled with medicine.

Read and reviewed the facts of query and reviewed them in context to your updated USG scan / LFT/ and CT Abdomen Report.
Based on the facts of your query,sudden presentation of Jaundice, non-responding to medicines has indicated grave situation already clinically.
This situation is lately confirmed by USG/BLOOD tests.
And is now re-confirmed to be grave with CT reporting,as the critical situation requiring Planned Surgical procedure to relieve the Obstructive Jaundice

Your aunty would need a major surgical procedure(?Pancreatico-Duodenectomy with CBD(Common Bile Duct) and Gall Bladder-Called as Whipples Operation) ,after stabilizing her disturbed liver and general condition.
Surgery and its outcome would depend how the patient responds to the stress of this Major surgical Procedure.

After reading your reply. I understand that This first needs to bring down the jaundice and then go for the major surgery.

How to bring down the jaundice?
Is ERCP stent is the only mechanism or medicine alone are can do this?
if ERCP is necessary, I read that steel stent will be the preferred one than plastic. But what is the life of the stent and how many years people with this stent can live?. What are the side effects?

if the body is not in the situation of supporting the major surgery, can she lead the life with the stent alone? With the reports that we had what is your opinion about her life span with out surgery?

My Impression of your case-

Hi Dear,Thanks for your updated query.
Reviewed your query facts with the Multislice CT reporting in context to the operability and resectability of the Pancreatic Malignant tumour in your case.


-Operate or not to operate-? Decide on the operability of Tumour in Your aunt's case-
Dear,Staging for the resectability needs to be done before any decision is taken about the operative treatment of your case.
AS the resectability of pancreatic head tumor in your case is yet to be assessed,which is the function of the attending CT radiologists.

-Reduce Jaundice to Improve Post-Surgical OUTCOME-
Along with supportive treatment with Medicines, Decompression of the obstructed CBD(Common Bile Duct) and Biliary Duct System is needed to Improve General Condition and reducing the intra and post-operative risks to your Aunty.
Hence Pre-Surgical ERCP Stenting needs to be done prior to further treatment in your aunt's case.

My Impression on Operability of your Case-
-You and your CT radiologist and operating Surgeon Team has to decide first on the resectability of the Pancreatic tumor which is 2.6 cm x 2.8 cm size.
As the portal veins and arteries are not infiltrated chances of your aunty
undergoing Whipple have increased.
-Survival without surgical treatment can be increased with neoadjuvant Chemo radiotherapy with ongoing Chemo treatment.
This would increase the operability ,in 22% of the operable cases of the total number of Pancreatic Head Tumor Patients,and so in your aunt's case.
Whether your aunty fits in that 20% or not will be decided by the expert doctors attending to her.

Prima Facie-
There is a chance of operability in your case and you should work with your doctors to aim for that.
Palliative care with ERCP stenting is the option after that if it is found non-operable
