What Do These Following Lab Reports Indicate?

Question: Dear Sir,

I have enclosed the path report ( Please refer Liver Function Test in that report), ALP is little raised. is this near to normal?, I have been to gastro and he has given me some medicine such as Heptral 300 & Udiliv 300 MG, My liver is slightly enlarged as diagnosed in ultrasound and little pain in right abdomen sometimes. I was taking drink but always in limit and in past one month I quit forever.

What is your opinion about the elevated ALP.

Hello,
Thanks for posting in HCM.
I went through your query and also the full report attached with the query.
As I could see yourall reports look in normal range only ALP is slightly raised and AST is just on border.
If it would have been just blood report then I wouldn't have worried about it.
But you also stated that your liver on ultrasound is slightly enlarged and you have occasional pain in right upper abdomen.
It could have been possible because of drinking. Some people have high tolerance some do not.
Now that you have stopped drinking from a month it could be possible that these reports are better than what it could have been if you had done it when you just stopped drinking.
As you mentioned that your gastroenterologist have already started you on the meds which are hepatoprotective so I would suggest you to continue taking these meds for few more days or till when they are prescribed to you
Repeat only the 2 tests which are abnormal alongwith serum bilirubin or better repeat whole liver function test after 15 days to see whether it has increased or got better.
As we do not know the values of your LFT before we can't tell whether this report is better or not than before.
Hence I would advice you to continue the treatment and repeat the test after 15 days and you can get back with me with the report or consult your gastroenterologist with the same.

Sir,

Thanks for the reply. I had also done 2nd LFT after 10 days ( 29th Feb'2016) where ALT & AST was normal but ALP was elevated. Please cross check with the first one and you will find that GGT is decreased which is 6 whereas previously it was 21 ( Such a big difference) . Is is possible.

Unable to understand the variation, also indirect bilrubin was slightly higher. The normal ranges are also differ from lab to lab so confused. Please advise.

On the advise of doctors, I had done all Hepatitis Test and found normal. Thier is no sign as of now for Jaundice etc.

Please correlate both the report ( Earlier & This one ) and kindly interpret.

Thanks for following up with me.
Its good to know you have done all the hepatitis markers and other things to find out any other reason for raised liver enzymes.
Ggt if low is not a problem. Higher ggt is a problem.
Mr. XXXXXXX in medicine there is a saying that never treat the reports, treat the patient.
We do blood investigations when we have some complaints and based on that as supportive help we treat the patient.
If you pick up 10 people randomly and do their lfts 50% of them will have higher or lower values.
Enzymes respond to our day to day activities, the food we take, stress we go through and other factors.
You have stopped drinking now, are taking the meds prescribed by your gastroenterologist and there is improvement in your clinical health and also your blood reports.
LFT varies from lab to lab and their controls also vary. Hence usually we tell patients to stick to 1 lab only and get your reports from them only so its comparable with previous reports.
But nevertheless now your alt and ggt are normal range and as the alkaline phosphatase normal range is different in both the lab values we still can see some improvement in reduction in value of the same.
Yes the values can change drastically in less time if proper treatment is given.
Still if you want to be sure and to have comparative analysis, you can repeat test from XXXXXXX asia lab as its for 29th feb and now its april. So I feel you can repeat lft again and see for the change it has.
On last note, if you do not have any clinical complaints of any sort then do not look at lab reports and focus on them. Clinical status of patient is most important.
If you want you can do LFT again now in one of the labs you have done prior and compare the reports to previous reports. Or you can share with me and I will help you interpret it for you.
