
What Do These Following Lab Reports Indicate?

I recently have done my executive Health-checks (1. Haemogram: Haemoglobin, Packed cell volume, R.B.C. count, MCHC/MCV/MCH, E.S.R, Peripheral Smear, Platelet Count, Total W.B.C./Differential Coun, 2. Fasting glucose, 3. HbA1C, 4. Urea, 5. Creatinine, 6. Uric Acid, 7. Lipid Profile, Total Cholesterol, HDL, LDL, Triglycerides, Total Cholesterol, HDL Ratio, 8. Liver Function Test, Total Protein, Albumin, Globulin, SGPT, Alkaline Phosphatase, GGPT, Bilirubin (Total & Direct), SGOT, 9. Routine Urine Analysis 10. X-Ray Chest PA-view, 11. ECG, 12. ECHO, 13. TMT, 14. USG of Whole Abdomen (Screening), 15. Eye Screening, 16. History Physical Examination, 17. Physician Consultation, 18. General Test: Stool for Routine Examination, x-ray Chest PA View, Anti (HCV) Rapid test. 19. Eosinophil Count). All reports attached.
One more think I have done CT scan of nose and "PARA NASAL SINUS" detected.
Now my question:
1) how is the overall reports specially heart? (I was told I had pre-diabetic, fatty liver, nose block)
2) Whats your suggestion in terms of cardiology?
3) How can I conclude the shortness of breathing was due to Lunge, heart, nose or other issue?

I carefully passed through your question and reviewed all your uploaded test reports and would explain as follows:
1- Your cardiac stress test has resulted positive for cardiac ischemia. This means that further tests are needed to investigate for possible coronary artery disease:
a) Stress echocardiography
b) Coronary angio CT scan to evaluate your future risk for possible coronary events
Coronary angiography(coronarography) would be the golden standard test to examine the blood flow in your coronary arteries and detect possible arterial stenosis.
I recommend discussing with your doctor on the above tests.
2- Regarding your other performed tests, I would explain that your HbA1C levels and post-prandial sugar levels are slightly elevated. This means that your are in a pre-diabetic patients. At this stage some modifications to your diet (diabetic diet) is needed to have a better control of your blood sugar and avoid progression to diabetes.
Your sinus CT scan has revealed only chronic changes due to allergic reactions or chronic sinus infection, but there is nothing serious to be considered. I recommend continuing your actual medication (Montek). Allergy tests would be helpful to detect for possible trigger factors of this chronic congestion. Avoiding allergic triggering factors would help reduce your symptoms.
Your liver is slightly fatty and with increased dimensions. This has also caused a slight increase in ALT and bilirubine levels. Nevertheless your blood lipid profile is unremarkable.
For this reason some lifestyle modifications are also necessary to have a better control of your blood glucose and reduce the progression of your liver condition:
- reduce saturated fat in food and also sugar. Diabetic and low caloric diet is very helpful.
- Perform a lot of physical acitivity (walking, aerobics, recreational sports).
- Try to reduce weight. You are overweight (BMI 28.4). Loosing some pounds will help you manage better your blood pressure, blood glucose and reduce fat in your liver.
Regarding the shortness of breath, it is important performing the above mentioned cardiac tests, in order to exclude possible coronary artery disease. You should know that in diabetic patients, shortness of breath is a sign of coronary artery disease (even in the absence of chest pain).
Your lungs have a normal structure and function and your symptoms are definitely not related to a lung disorder.
You should discuss with your doctor on the above issues.
