Question: Please provide an opinion over attached discharge summary. Known case of Sarcoidosis since 1995. In 2015 DM due to Wysolone induced

His presenting symptoms, Auscultation findings and chest x ray report is suggestive of pneumonia.
Pneumonia is further confirmed by gram positive cocci in chain.
So patient was treated with vancomycin (antibiotic) for five days.

I can understand your concern.
Honestly speaking, it is very difficult to say accurately about all these.
In sarcoidosis, prognosis is usually good.
We need to keep sarcoidosis in remission stage.
If it flared up then hospitalization is required.
For remission, immuno modulator drugs like steroids, azoran, HCQs etc are required.
After acute worsening, we need to taper these drugs and continue one or two drug in maintainance phase.
Since these drugs alter immunity, there is always risk of infections. And hence infections are the most common cause for acute worsening in these patients.
So you can expect 1-2 admission in 1-2 years.
Overall life expectancy is good in sarcoidosis.
