What Do These Lab Reports Indicate?

Question: Hello Sir,

Trust this finds you well.
In continuation to the followup for my father please find the latest test reports attached after three months interval.He did complete renal package three months back in XXXXXXX i.e in August.I have sent you the same.Since you were not available some other doctor (Dr. Rakesh) answered.Hope you are able to access the same reports.

Kindly suggest how is it placed now? Few other questions that we have are.

1.Previous reports says "bilateral kindneys show grade -11 echotexture with lobular margin".what does it mean? what it's impact?
2.My father was using cremaffin (liquid paraffin) 7/8 years back when he has piles problem.Is this echotexture because of cremaffin use?
3.We were also assuming the current issue could be because of hypertension.Is it true?
4.Is there any diet control for now?

Hi.
I am good and thank you.

I have seen the latest reports of your father. It appears that he has Chronic pyelonephritis, which is the consequence of the past several urinary tract infections. Lobulated and echogenic kidney suggests that he had scars in he kidney with which the locally there is some shrinkage of the kidney leading to lobulations ( irregular outer margins of the kidneys). Creatinine is also elevated suggesting that he has mild kidney dysfunction. Cremaffin cant produce that but cremaffin which has magnesium which is already supposed to get elevated in kidney dysfunction> so you need to stop cremaffin.

Hypertension is the consequence of such scar(s) in the kidney rather than the cause. He requires therapy not only with anti hypertensive drugs but also with sodium bicarbonate therapy.

Being elderly, we should not restrict so many things in diet. Conventional prescription of low protein diet would not be good to them. You can allow him lal vegetarian diet including dhal and avoid non vegetarian diet. He can take all varietis fruits in moderate quantities.

YOu don't need to worry about now. He has to maintain good general health and eat well. Let his all complaints be heard and attended to. let him be under regular follow-up. He must have been seen by some one before from nephrology. Don't worry. Get those test's along with few others at regular intervals.
