What Do These Rapid Typhi And CBC Report Indicate?

Question: My wife aged 44 has been feeling feverish since last 1 week. She had CBC & Rapid typhi done on 29th May'15.
Report:
TLC 5.8, Platelet 196000, Hb-9.7, Rapid typhi - positive. She is Thalasamia minor and Her Hb remains generally on lower side.
Took Levoflox 500 mg once daily for 4 days since 29th May'15. On 31st May'15, She gave blood for its Culture and LFT. Final Blood Culture Report shows no growth. SGOT - 27, SGPT-30.2.
She is still having Headache particularly in the evening hours, having nausea, feeling week & feverish, but thermometer shows no temperature except on 2/3 occasions around 98 deg F / 98.5 deg F.
Question: Has she been suffering from typhoid ? How to confirm it? When is Widal test done ? Should she start another round of Antibiotic or give Urine for Culture first. Her period is cleared today. Is it advisable to give Urine sample now ? Had Blood Culture test showed false negative as she had started antibiotic before that ?

Hello , thanks for your query.
We discuss the condition stepwise.
Firstly rapid typhi was positive , because In XXXXXXX typhoid is endemic means all of us have some kind of infection without apperent disease.
Second, blood reports appears normal.
Blood culture should be done in first week and widal in second week. Your investigation are done approximately.
At present there is no temperature/fever as per your thermometer reading. Most importantly blood culture is negative, which is definitive test to rule out infection.
Considering all this finding it doesn't look like that she is having typhoid.
I will advise just wait and start healthy diet so that she improve. I don't think need of further course of antibiotics .
Continue monitoring temperature and if fever reappear then consult your physician again.
He/she is evaluating in proper manner so don't worry.

She is still feeling severe headache during evening hours, nausea sometimes and feverish. In the night time she has to tale Calpol for Headache. Temperature is not coming in thermometer. She has been advised Cefixime 400 mg twice a day for 5 days. I am in dilemma, whether she should start Cefixime. If Widal test is to be done then can it be false negative due to start of antibiotic.
She is already on Darolac & Cobadex forte.

Hello, welcome back.
If antibiotics are started prior to any test which detect antigen or antibodies, then such test could give false results.
However culture result will be confirmative in such cases. In our case culture is negative , so chances of typhoid ruled out.
If she has been prescribed to certain antibiotics then please continue it and do complete course.
It will take some time for complete relief. Rest of tablets are for symptomatic relief like headache.
