Question: Hello Doctor - This is a critical issue to us and please assist us in clarifying our queries.

4. NORMAL DOPPLER STUDY. NO EVIDENCE OF FETAL HYPOXIA/ ACIDEMIA SEENT AT PRESENT.

SUGGEST REPEAT SCAN AFTER 2 WEEKS FOR INTERVAL GROWTH.

During 1st trimester - Weight dropped to 71.5
Later, weight grown consistently 5th month to 75.9

In Sixth month end, suddently 4kgs increased to 80.6
Doctor suggested, eat frutis,vegetables and maintained same
weight till end of 7th month,no gain in weigh. ie.
both 6th month end, 7th end same weight.

Queries:

1. Does maintaining same weight during the 7th month continuously has resulted in this situation of slow growth?

2. Doctor said 1.15 percentile is less number and if it continues like this, we have to go for early delivery in next 2 to 4 weeks. But when I searched in internet, i noticed number of people who had the same issue, waited till 35- 36 weeks and noticed some improvement overall and later they went for c-section.
a.Going for delivery in next 2 -3 weeks is correct ?
b.Can we wait for some more weeks to see improvement in growth?

3. Baby head size is fine and abdominal growth 1.15 percentile. doctor said, as the growth is slow, more blood flows to head and so its size is fine.
a. How to improve the growth of abdomen?
b. will taking more nutrient food will solve this?

4. Because growth is slow, she gave betanosol(steroid) injection for 2 days,2 dose at 31 week within 24 hours. I got to know betanosol injection is not good for children in internet and it my hamper their learning skills in future.
a.Is it safe?
b.will it effect my child?

5. Doctor prescribed ARG9 sachets(1-0-1) daily for 2weeks until next scan. after 7th month, we started having good volume of food.
a. How ARG9 helps this to treat this issue.
b. Will weight gain will treat the slow growth issue.

Hello Sir, thanks for trusting health care magic.
I understand your anxiety related to the issue.
There is indeed nothing to worry in my opinion and your gynecologist is walking too safe and going by the investigations only.
There is practically no sign of growth lag and the difference in gestation period observed is just an observation bias which is common and allowed up to 2 weeks of difference.
Points in favor of normal growth are -
- normal liquor pocket
- normal grading of placenta
- normal Doppler
- normal biophysical profile.
Only small growth difference in gestation and AC is a very crude thing to rate the baby as growth retarded.
Its alright if she has been given betnesol as it is good for the baby and is commonly administered so don't be anxious about it.
Keep taking ARG 9 as advised as it is very good a supplement and thus will have only positive effect on growth of the baby.
No need of immediate delivery after 2-4 weeks and can well wait for full term but going for regular doctor visits and taking all the medicines.
As far as mother's weight is concerned ask her take ample amount of water, milk, fresh fruits and vegetables and dryfruits like almonds and walnuts.
She will surely gain weight properly and also measure it on a home machine weekly in case possible as machines in hospitals have a lot of variation so cannot be graded precisely.

Thanks Madam for understanding our pain and for for your prompt response.

She is much worried about the betanosol injection which is a steroid. Will it hamper child's learning abilities in future. Or is there any other side effects to baby now & then in future. If so what we can do to take care of it. more anxious about this.

a. Does the injection helps in growing baby in gaining weight apart from lungs maturity?

b. It is mentioned in reply that, this injection is commonly administered. in which kind of common cases, it is administered.

c. Also, I have attached the growth scan report that has growth trend of MCA RI, MCA PI, HC/AC graphs. Please give your opinion abt this also. Thanks in advance.

Very helpful an injection

You are most welcome and her anxiety is genuine and completely understandable.
Betnesol injection has no side effect on the baby and is commonly given to almost 10-15% of pregnant women mostly in those who are at risk of preterm delivery due to any reason.
It escalates lung maturity, helps in maintaining integrity of gut structure of the baby and in general promotes the general wellbeing of the baby.
So don't keep any reservation against this injection as there are upcoming recommendations to give it to all pregnant females irrespective of any indication.
I have seen the fresh reports too and suggest to stay relaxed as all reports are normal and there is nothing that needs active intervention in a week or so.
All u want to add is to get a repeat Ultrasound and Doppler after 2 weeks and keep taking rest and all medicines prescribed to her.

Thanks Madam for clearing our queries clearly.

Regarding food intake, doctor advised to go for fishes. So far these days, I have avoided them due to high mercury content. Instead we went to have
walnuts more in a day.

Can you advice which fish is better to go and safe during pregnancy time?

Fishes on my list:

If fish has to be taken, what quantity per week is suggested?

Also, as u have reviewed our case, can we have more mutton than chicken to gain weight & protein ?

As far as mutton or chicken is concerned, mutton is better in case she likes it.
Fish is definitely good and can be consumed on weekly basis and for that both salmon and pomphret are good. They are very rich in essential fatty acids so good for brain development of the baby and thus very good in pregnancy.
But for growth mutton is best.
