It could be due to some technical problem the biopsy could not show the testicular tissue, hence a repeat biopsy was requested by the pathologist for further evaluation.
Since the ultrasound scan shows bilateral tiny epididymal cysts, so it seems to me, apparently you have Azoospermia of obstructive type.
Regarding the cost of the treatment it varies from center to center, it may cost around 2 - 3 lakhs in a specialised IVF centers, however you may consult your treating doctor who is more familiar with the costs in various referral IVF centers.
