What Do These Ultrasound Report And Color Doppler Test Result Indicate?

Question: Please route to Dr T chandrakant as he is already helping me in the issue, this is a follow up with him on further after the reports, please refer previous questions closed .

I am attaching the latest report on my case, i have met another family practiconer and he did a Ultrasound and blood test, he is of the opinion that it is inflamation and wait some time, blood test is clear of no infection and hence i can wait for 10 days atleast

Questions: 1. Dr T chandrakants views after the report, esp can i wait few more days 10 days without travelling XXXXXXX .

2. Any comments in blood results

Looks to be recurrence of fistula, blood reports are normal.

Hi.
Thanks for your review questions and the attachments.
Gone through the reports of blood and ultrasound report and the opinion of your new Doctor.
1. The Ultrasound report with color doppler is suggestive of a fistula in ano, meaning a recurrence as there is a probable communication with anal canal at the
level of 3 cm from the anus.
Simple test of injecting Betadine with hydrogen peroxide in the outer wound and having proctoscope in the anal canal and rectum can confirm whether this is a fistula or a sinus (not communicated to anal canal or rectum). Even the internal opening can be felt and seen.
Or you can ask your new Doctor or the Radiologist about fistulogram which is confirmatory.
If this is so, there is a need for re-do for such a recurrence.
Even if it is a sinus, needs scrapping.
Since there is no acute infection causing any local changes or change in blood reports, you can certainly wait and there is no need to rush through.

2. The blood reports are within normal limits. Since the tract is opened one, the chances of causing acute infection or inflammation are rare. Hence no need to rush through.

Thanks doctor for the detail reply. i am currently working in vietnam and the medical doctors are still not the level of XXXXXXX international standards . hence instead of dragging with another test , i am planning to travel XXXXXXX XXXXXXX on May 8 after 10 days and do detail test and if needed do the surgery
For the test, i am planning in Cochin XXXXXXX I am thinking of Aster Medcity, gastroenterolgy surgeons, cochin is my home town

I cannot find any specilised colon rectal surgeon, in XXXXXXX ,but is it a correct understanding that gastro surgeons are same as colon rectal surgeons for this. any other advice you have for me for treatment option in XXXXXXX

Colo-rectal Surgery Specialist is one who has more interest into this field and works in this area the most. He can be a General Surgeon or a Gastroenterology Surgeon.
What matters is his mastery over the subject, the type of the fistula whether low anal or high anal and so on.
In India: let us see the report of whether this is a fistula or a sinus, the type (low/high), Histopathology report of the biopsy of the tract whether this is any specific infection like tuberculosis or so. All these factors will decide the best possible curative treatment.

I am planning to XXXXXXX on May7, i have a questin, in my last question , on 21.4.2016, the surgeon here said that he dressed the wound, opened the wound to remove excess growth on floor of wound. This process took some 10 min to complete and was did on a local anaesthesia sprayed in anal area. My question is by any chance the inflammation now seen in ultrasound scan is related to this wound opened.when i go XXXXXXX will the doctor get confused over all these things, during mri is it be able to pick the root cause of issue, inflamation, healing process or new fistula tract etc?
do we need to wait more time since the open wound process is done recently ie 9 days, to allow it to heal , if that impacts inflamation.

Hi.
Read your follow-up.
The inflammation seen on ultrasound may not be related to any of the procedure done as you have noted as this wound has an opening.
Secondly, this is the area that is always exposed to movement as we walk or for that matter any movement and also sitting up and putting pressure makes it difficult for healing to occur.
Ideally speaking there should be no confusion as the MRI/ Fistulogram by conventional contrast XXXXXXX in the tract and X-rayed and the simple tests of injecting the mixture of betadine and hydrogen peroxide into the tract and watch from inside with a proctoscopy confirms it all.
No need to wait for the investigations to know whether this is a sinus or a fistula.
If fistula- whether this is high or low anal as this is the most decision making factor hence try to get these done as soon as possible.

Please feel free to ask anything and any/many times till you are assisted well till cure.
In high anal fistula the Kshar-sutra method works the best, only factor is the thread needs to be changed every week or so.
