What Do Violent Outbursts And Homicidal Thoughts In A Child Indicate?

Question: A lady I work with has had to take care of her niece because she grew up as toddler in a disfuctional home. My friend and her husband have taken care of her for the last 5 years. She has had 6 hospital stays during that time period. She had another violent outbreak yesterday and my friend is losing it! What does she do?
Brief Answer:
Need more details so that I can better help you.
Detailed Answer:
Hi,
Please describe her symptoms in detail, like since how long she has the symptoms?
What is her daily routine?
What does a violent outbreak mean?
Did she hurt herself or others?
Her sleep pattern?
Her socio-occupational function?
Hope I have answered your query. Let me know if I can assist you further.
Take care

Ok, she has breakdowns when she doesn't get her way (nearly daily). She hits walls, doors, clears tables, she will hit herself in the head or bang her head on the wall over and over, and recently she has started hitting us or elbowing when we try to restrain her.
She walks and talks in her sleep even though she is on Trazadone nightly.
She has shown symptoms since she was around 4 (adopted at age 3.5) started out with tearing up her belongings and urinating in her floor.
She is a very smart girl but fails at using it to the best of her ability.
She has threatened to kill us twice. Once she stated she was going to do it in our sleep with a knife (hospital stay number 6) and prior she didn't have an actual plan. She has climbed out the window and gone down the street several times but has never really gone far. Takes about 2 hours to get her calmed down and back in the house.

Brief Answer:
My suggestion.
Detailed Answer:
Hi,
I'm sorry for the late reply.
I have gone through your details thoroughly and can understand that definitely, she has behavior problems.
If she is completely normal until the age of 4 yrs, then this behavior could be due to either psychotic or bipolar symptoms.
She needs to be treated with antipsychotics like risperidone and also mood stabilizer has to be added.
I hope she was already given this treatment and there is every necessity to continue this, and maintain good compliance to avoid flare-up or relapse of symptoms.
If still, she has violent behavior, she needs to be hospitalized again.
I would advise you to consult your doctor regarding the immediate intervention and the requirement of hospitalization based on the severity of symptoms.
Hope I have answered your query.
Take care
