What Doe Abdominal Pain In Child After Food Suggest?

Question: 12 yr old with constant abdominal pain. Especially after eating. Feels need to use restroom within minutes of eating. Has both diarehea and constipation. Blood work normal, CT normal except fluid in pelvis and 12mm lesion on L2

With these symptoms like altered bowel habit,pain in abdomen and urge to defecate just after feeds I would like to rule out Irritable bowel syndrome.
Earlier it was thought to be a disease of adults or elderly but now a days it is common to see such patients in the pediatric OPD.
The underlying cause may be bacterial infection,small intestinal bacterial overgrowth,allergy to some cereal's protein (like wheat,rye or barley and the condition is known as celiac disease) or some psychiatric etiology.
The psychiatric component is much less common in pediatric age group.As in most of the cases there are no structural changes, the CT or other imaging comes to be normal.
Some investigation should be done related to the etiology like stool test and celiac screening.
The management includes medications,lifestyle modification and dietery regulation.
I would like to give some antibiotic like ofloxacin and ornidazole/ciprofloxacin and tinidazole or rifamycin as it will cover the infection if any.Along with this probiotic should be given.Antispasmodic agents like drotaverine or dicyclomine helps if there is severe pain.Please get it presribed by aphysician as all these are prescription medicinesFiber supplements like Isabgol helps in relieving the symptoms.
Large meals can precipitate the intestinal spasm and thus pain.Giving small frequent meals is a better idea.Avoid milk and dairy products,carbonated beverages,fatty and spicy foods,junk foods,coffee and beans.
Doing regular excercise,drinking plenty of fluids and participating in sports helps a lot.
After going through the question and answer history I agree with the radiologist and suggest you to follow them for the bony lesion.
I hope after following all these measure the symptoms will improve.
