I have noted your symptoms and investigation reports.
First of all, I would like to reassure you that there is no need to worry as you do not have any serious neurological problem.
You have symptoms of TIA- transient ischemic attacks- leading to the current symptoms. Aspirin should be continued. I usually give aspirin at a dose of 325 mg per day.
In addition, if you get frequent headaches, you should be started on an anti-migraine drug to reduce the frequency and severity of headaches. These include topiramate, divalproex, etc.
40% stenosis of vertebral artery on MRA is not significant and aspirin would take care of that.
You should also be on statins to keep cholesterol within normal limits and prevent further TIAs.
