What Does GFR Of 54 Indicate?
Kidney disease....
Detailed Answer:
Hi,
Before running into final conclusions, may I have the full report of your blood work? I need to see the reference range of your lab.
GFR 54 : is indicative for kidney suffering (possible kidney failure). Normally, GFR should be higher than 90 is considered normal. Eventually, your kidney are suffering. To my opinion, a systemic disease might be a cause.
As you have a previous history of cervical cancer B1 positive with pelvis nodes and pelvic external and internal radiation around 12 years ago, and now you are presenting with fatigue, weight loss and pain on right quad and minor hip bone pain both sides; all these, make me think over possible relapse of cervical cancer that should be further evaluated. My doubts go for possible meta in the kidneys that should be excluded.
Absolute Lymphocyte 4.1: It seems to be within normal range, how, should have the reference range of your lab.
Hematocrit 46.7%: It seems slightly elevated. I need to have the reference range of your lab too.
WBC 17.8 and Absolute Neutrophil 12.6 are increased which is indicative for possible infection/inflammation or bone marrow problems due to radiation.
Hemoglobin 16.2 seems to be at upper normal limit. The reference range of your lab is needed.
Platelets 155: are at lower normal limit.
RBC 4.91 is also within lower normal limit.
Your blood pressure is within normal range.
I understand the blood results are causing stress and anxiety to you. What I find reassuring is that your hemoglobin, platelet and RBC are within the normal range although at the upper and lower limits respectively.
With regards to WBC and Neutrophils, they are indicative for possible infection. With a GFR of 54, most probably, kidney disease is the cause and should be further evaluated.
As you have a previous history of pelvic radiation, bone marrow should be also checked.
Apart OG consultation, I advise to get it checked with Hematologist (specialist for blood disorders) and Nephrologist (kidney specialist).
Wish you all the best!

I understand the GFR number. I understand the bone marrow possible issue. Hoping not met disease/recurrence obviously.
Kidney issue from radiation more likely? or recurrent maybe? Leukemia? Lymphoma? Yes, I am concerned, but calm and waiting for Monday.
I am expecting to be told on Monday to see a Nephrologist and Hematologist (oncology one?).
Thank you so kindly.
Possibly uro-genital infection,need to confirm...
Detailed Answer:
Hi back XXXXXXX
Thank you for providing your test reports.
I am very relaxed now that I have gone through the complete report (including reference range).
So, GFR range is higher than 60, while yours is 54 (a slight decrease). As you are having high WBC with increased Neutrophils, most probably you are having an uro-genital infection.
I was looking for urine test reports, but did not find any. That will determine if there is urinary tract infection (UTI) or vaginal/genital infection.
Next, as per bone marrow depression/issues, you are only having changes in some blood count elements and not all of them. To ensure we are not having bone marrow issues, hematologist consultation is needed.
As per oncology consultation, if I was your caring doctor would ask to have a consultation with nephrologist, hematologist and OG specialist first. If, after this consultation, they would ask for oncologist advice, then, can schedule appointment with oncologist.
For the moment, these are specialists I would advice to get consultation together with urine analysis and vaginal swab testing. Although your cervix is gone from radiation, your vagina is still there, and might get infection too. PAP test will determine abnormal cells, it is not specific for infection.
I do not want to precipitate into final conclusions. Medicine is a mystery equation that needs to get resolved step by step without being stressed unnecessarily. We are here to help and give second opinion regarding your health problems online.
I like the fact that you are waiting for the Monday appointment calmly. These are routine examinations that should be performed in patients with your medical history. Once you have the results, can analyze all of them.
All the best!
