What Does RBC Count In Time Indicate?

I have a follow up appointment with my doctor to go over labs. However, I noticed that my eGFR has dropped 25% over the past year. It went from 84 to 63. I am a Caucasian female. My creatinine is above normal limits.
In addition, my urinalysis showed a high number of RBC, hemoglobin, and occasional free fat. I had it retested two days later and the RBC were no longer elevated, but there was still hemaglobin present as well as occasional free fat.
I am not menstruating or even close to the start of my period. I do not have any vaginal cuts or trauma to explain the blood.
Are these significant findings? If so, how significant do you think they are and what could they indicate? Also, what would you do next to further evaluate if necessary?
Thank you so much!
I wanted to add a little more info - I actually got the time and % decrease wrong. My eGFR has actually decreased 28% in 1 year and 4 months. Furthermore, it decreased 19% just in the last month and a half. Here are the numbers:
2/4/17 - 87
5/7/18 - 77
6/20/18 - 63
I was well hydrated for each test.
Rbc in urine can indicate nephritis or urological illness
Detailed Answer:
Hi,
RBC in urine is not a good thing because it can be a marker of kidney inflammation as in a case of nephritis . Your doctor can order urine protein creatinine ratio and serum creatinine levels to rule out nephritis.
RBCs in urine can also be because of urological problems which would include genitourinary infections, tumors or other cystitis another illnesses. these can be diagnosed with urine for malignant cells , urine for AFB and imaging studies like ultrasound and CTScan.
Weight loss is not a good thing. unless it is part of a planned diet or exercise schedule. it also signals presence of disease in form of chronic infections and malignancy. so needs investigation. hyperthyroid should be ruled out. in case of renal disease you may also have high bp and swelling of legs.
In a nutshell, you have a significant history and only way to diagnose the problem is by following advice of your treating physician as you yourself will not be able to narrow down the possibilities.
best wishes
