Question: my friend is 42 yrsw. old , not overweight and his blood pressure is 138 over 107

I have gone through your query and I can understand your concerns.
The blood pressure recording of your friend can be considered as borderline according to latest criteria.(normal being 130/90 for his age)
The diastolic part (second portion of the recorded BP) little bit high.
For people in the age group of 30-50 140/110 for a short period of time is not dangerous. Antihypertensives is not necessary and there is no need of hospitalization. But keep in mind that this could be a feature of early hypertension so frequent monitoring is necessary . If such BP level last for a long time,you need to take medicines to lower blood pressure .
As of now, non treatment is needed. Try to lower salt intake. Avoid salt rich good stuffs like pickles and cheese. Quit smoking and do regular exercises.
