What Does A Large Soft Lump In The Testicular Sac Indicate?

Question: I am a healthy, 72 year old male. I recently noticed a large, soft lump inside of my testicular sac, almost like a third testicle. It is not painful, but I have also noticed that I have been feeling a little nausea. The lump doesn't appear to be directly connected to the two testicles.
