What Does A Mass Around Abdominal Aorta Indicate?
Question: I have been diagnosed with a mass growing around my abdominal aorta; 9cm long and about 7cm wide. Had a needle biopsy performed Tuesday of this week. No pathology report yet. The mass is a soft tissue mass.

There are many possibilities to discuss regarding this mass around abdominal aorta. Can you please provide the full report of the tests you had? I need to know :
- in which segment of abdominal aorta the mass was located?
- any other pathologies of nearby organs seen?
There are many organs nearby abdominal aorta, the problems of which might have caused the mass. So, it is important to understand if the mass derives from the organs nearby or from aorta itself.

There are many organs nearby abdominal aorta, the problems of which might have caused the mass. So, it is important to understand if the mass derives from the organs nearby or from aorta itself.

Indication: Abdominal mass,. History of gastric bypass surgery, cholecystectomy, and hysterectomy.
I have not received results yet from needle biopsy. CAT scan/chest shows no evidence of metastatic disease/lymphadenopathy in the chest. A large, bulky retroperitoneal soft tissue mass is present that surrounds the aorta on all sides except posterior to the aorta as described. A couple other enlarged retroperitoneal lymph nodes are present. The differential diagnosis includes lymphadenopathy/lymphoma, retroperitoneal fibrosis, and malignant fibrous histiocytoma. No evidence of metastatic disease/lymphadenopathy in the pelvis.
Heart size is within normal limits. No pericardial effusion is identified. The thoracic aorta is normal in caliber. There is a small sliding hiatal hernia. The trachea and thyroid gland are unremarkable. Lungs: No pulmonary, focal airspace consolidation, or pleural effusion are seen. Chest wall: Moderate degenerative changes of the thoraces spine. No suspicious osteolytic or osteoblastic bone lesions are identified.

For your information, retroperitoneum is the part of abdominal cavity located in front of spinal cord. Apart aorta, it consists of lymph nodes, connective tissue.
The mass noted around aorta is probably an enlarged lymph nodes (as the report has shown other enlarged lymph nodes). No suspicious mass in pelvis, chest and pleural effusion noted. This is a good finding that suggests the problems are localized within the local lymph nodes and not generalized.
Therefore, to my judgement the mass is related to the lymph nodes, most probably lymphoma. I advise to have patience until biopsy results are in for further evaluation.
I also advise my patients experiencing the same situation as you to stay calm and wait for the biopsy to decide then, for the most appropriate treatment strategy.
