Brief Answer:
an infection is most likely

Detailed Answer:
HI, thanks for using healthcare magic

It is likely due to an infection, infection can be viral, bacterial or fungal most commonly.

Viral:wart, bacterial: impetigo , fungal: tinea capitis

These are the common infections that can involve the scalp. All can cause infections that can persist for a long period of time.

A wart would be a raised hard lesion. That would recur, based on the fact that you said that it was hard, this may be the most likely

Impetigo is a bacterial infection that can occur on any aspect of the body, would require treatment with antibiotics. It is contagious. Pus or discharge can be seen though this can also be seen in fungal infections.

Fungal infections are also contagious, treatment requires oral anti fungal medications for 2 to 3 weeks.

Diagnosis would require someone taking a look at the lesions. Unfortunately most skin lesions require visualization of the lesions for diagnosis.
