
What Does An Abnormal ECG Report Indicate?

As part of Regular yearly checkup ECG and TMT reports have been taken, it seems there is some problem in the reports (as it has been written as upnormal ECG), so we went to a doctor and he told there may be minor problem prescribed two medicines Ecosprin and Macpril as mentioned below,
Please suggest whether there is any problem in the ECG and TMT reports also pl suggest what we have to do in this regard.
Is there anything serious. Pl do suggest if there is any restriction in the food we have to follow.

TMT is suggestive – not conclusive of – Infero-lateral ischemia
He needs to be investigated further for Coronary Artery Disease... especially because of the family history...
Absence of symptom is no proof of absence of disease
The next step is TMT with thallium isotope. It is the ideal non-invasive way to evaluate ischemia / to assess the PHYSIOLOGY (function) – to see whether the blood arriving at the heart muscle.
If the test is positive, the next step is to see the ANATOMY (structure) – undergo catheterisation and coronary angiography with a view for possible intervention. It is the only way to directly ‘see’ the block, if any – and its location, extent, severity and so on. Coronary arteriography is invasive but it is the gold standard for this. / CT angio is non-invasive study for the anatomy. If positive, he will need catheterisation, anyway....
Go ahead and get him investigated / Better be safe - treated if a problem is found / Happy, if it turns out otherwise.

As already informed, I want to add here that at present my husband is not having any problem.He is following healthy diet with regular exercises.
Please do reply for the following queries also:
1.Whether the medicines as prescribed by the doctor can be started.
2.Whether we have to take the TMT and ECG tests once again before taking TMT with thallium isotope.
3.Why it has been mentioned as upnormal ECG in the reports.
4.Is there anything serious to worry since we all are worried about the report.

1. Yes – the medicines can be started / and continued doubts are cleared by further tests.
2. Go for TMT with thallium – no use of repeating the plain stress again
3. There are certain changes (ST segment-T wave) seen in the test – especially at peak exercise – that is why, it is repeated as abnormal
• It could be a false positive/ it may mean underlying block in blood vessel
Only further test can clarify the doubt

4. Also please mention whether we have to consult a cardiologist, since we have seen only the general doctor

1. As soon as possible – the aim of any investigation is to modify the treatment, based on the result / Appropriate treatment demands accurate diagnosis / and that demands specific investigation
2. No food restriction – it is dictated by BP, glucose and lipid levels
3. No restriction to routine day to day work or life / No dietary restriction now
4. Cardiologist consultation is a must
5. No tobacco or alcohol
No tension –‘Take it easy’ policy
Regular exercise – at least half an hour a day / at least five days a week

As per your reply we have consulted one cardiologist and he suggested to take the medicine Ecosprin AV 75 one per day at night. After some time we can go for ECHO, if required.
As replied earlier proper exercise he is doing,
at-last please tell me some tips at this for my husband as a XXXXXXX cardiologist with your experience.

LIFE STYLE MODIFICATION IS THE BASE –
WEIGHT – to be reduced / he is overweight / His BMI is 29
BMI for Normal weight: 18.5 - 24.9 / Overweight: 25 - 29.9
DIET
Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / XXXXXXX sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.
You should see a good DIETICIAN - One generally needs SEVERAL sessions, to master concepts of calories, nutritional values, biological value, vitamins, minerals and so on.
EXERCISE as suggested
NO tobacco or alcohol or tension - as already said

ECHOcardiogram is done to see the heart valves (Mitral Valve Prolapse) / heart muscle (Cardiomyopathy) and heart function (Ejection fraction, wall motion abnormality) / clots... Here, it is likely to be normal / but is ROUTINE, non-invasive, repeatable / SHOULD be done.
Ecosprin-AV contains aspirin and atorvastatin – aspirin is anti-platelet and helps to reduce clot formation / atorvastatin is for cholesterol but also has other helpful effects... they should be continued for life / they are taken for Primary Prophylaxis / but also by many normal people without any problems, from forties and fifties
The ideal option is to go for TMT with thallium isotope. It is the ideal non-invasive way to evaluate ischemia / to assess the PHYSIOLOGY (function) – to see whether the blood arriving at the heart muscle - treated if a problem is found / Happy, if it turns out otherwise
