I am sorry to hear that your husband has been having red and painful scrotum. In light of his recent use of doxycycline which kills bacteria I can safely say that he has a fungal infection now in his private areas. This is a common reaction to antibiotics. Just like women can get vaginal yeast infections after antibiotic use men can get yeast infections on their private areas too. The use of lotions and creams and especially ointments only makes the fungal infection harder to treat.
I recommend the following
1. when bathing use a mild soap and water to the genital area
2. dry gently but thoroughly
3. use an antifungal spray or powder like tinactin or lamisal. Do not use a cream
4. wear loose fitting underwear or allow the area to be open to the air.
5. Keep the groin as dry as possible. If he has problems with incontinence and wears diapers then change them frequently
6. If the redness does not improve then he may need an oral antifungal like diflucan
