Brief Answer:
I would explain as follows:

Detailed Answer:
Hello!

Welcome on HCM!

Regarding your concern, I would like to explain that the above mentioned BP (197/140 mmHg) is an extremely high value and needs an immediate medical intervention to restore blood pressure within the normal average ranges.

This needs to be done to avoid any potential life-threatening complication (stroke, myocardial infarction, pulmonary edema, renal injury, etc.).

A careful BP values monitoring needs to be done after that in order to properly conclude on the eventual maintenance antihypertensive therapy.

Additional medical tests should be done for correctly judging on possible predisposing factors and selecting the right medication scheme:

- complete blood count,
- BUN & creatinine,
- blood electrolytes level,
- AST & ALT,
- bilirubin,
- fasting blood glucose level,
- resting ECG,
- chest X ray study,
- cardiac ultrasound

You need to discuss with your attending physician on the above mentioned issues.

Hope you will find this answer helpful!

Kind regards,

Dr. Iliri

Opinion as follows:

Detailed Answer:
Dear XXXXXXX

140 is the lower value and 197 is the higher value corresponding to diastolic and systolic BP values respectively. Whichever is their order the meaning is the same.

So, if you provided correctly the BP values the above reasoning is true.

As you are actually on antihypertensive treatment, it is necessary to closely monitor the BP values for a couple of days (several times daily) and let your doctor review those values (also you can discuss them here on HCM) for deciding whether a current therapy optimization is necessary.

Meanwhile, I encourage you to follow the above mentioned recommendations (including medical tests).

Wishing you a good health!

Brief Answer:
Please try to focus on the importance of high BP risks.

Detailed Answer:
Hello again XXXXXXX

Let me explain you that 140 over 197 mmHg means that the lower blood pressure value is 140 mmHg and the higher value is 197 mmHg.

In medical literature and all over medical discussions the right way to declare a BP value measurement is:

Systolic value (higher value)/Diastolic value (lower value) mmHg, that is 197/140 mmHg.

Nevertheless, whichever form you prefer to express your BP value, the medical meaning is the same, your BP values are abnormally high and further consideration is needed.

You should believe me, as I am just trying to explain the importance of controlling hypertension due to its dangerous complications.

I strongly recommend you to follows the advice I mentioned at the beginning of our thread (frequent blood pressure measurements and additional medical tests).

Regards,

Dr. Iliri
