Question: my wife is 50 years old and has type 2 diabetes. she is jittery and weak. her sugar is 229 but her blood pressure is 80/50. is this something to worry about?

This situation could be related to dehydration, anemia or an electrolyte imbalance. Diabetes can also cause autonomic nervous system dysfunction, leading to orthostatic hypotesions, which can also explain her symptomatology.

Coming to this point, I would recommend giving her plenty of fluids to remain well hydrated. A coffee would also help raise her blood pressure values. She should stay at rest and avoid physical activity or standing up in the next hours to avoid the possibility of falling (low blood pressure can lead to fainting, falling and low blood perfusion).

If this situation persists in the next hours even after the above measures, I would recommend bringing her to the ER for a careful physical exam and some tests:

- a resting ECG
- fasting glucose
- blood electrolytes for possible imbalance
- complete blood count for anemia
- kidney and liver function tests.

She may need to take intravenous fluids if the situation persists.
