What Does Borderline Right Axis Deviation And Anteroseptal Infarct Indicate?

Posted on Wed, 10 Sep 2014
Question: I recently went to the ER to check on an odd palpitation after working out, and my ECG came back abnormal. I made an appt with a cardiologist but wanted some feedback before my appointment. My sinus rythm is normal, but have borderline right axis deviation, and anteroseptal infarct, old!! ECG was abnormal at the bottom of the page.

I was looking this up and it says I had a prior heart attack??? The ER doc did 2 ECGs and the same results came out. Please advise on what you see on my ECG, I will attach. Thank you,
doctor

Brief Answer:
not exactly abnormal ECG, an Echo should be done

Detailed Answer:
Dear Mr XXXXXXX

You have normal sinus rythm on your ECG. Borderline right axis deviation is not abnormality, it can be constitutional.
The report was prior heart attack due to small R waves in leads V2 and V3, but actually it's not seems like an old heart attack, it also can be constitutional. And sonetimes there are lead malplacement.
So you should not worry. ECG has only 60-80% sensitivity.
You should wait until your appointment. An echo will be done. And I think that everything will be OK.

I hope I could help you.

Can you please clarify what you mean by constitutional? Meaning normal? I have an appointment today with a cardiologist today and will do more tests. Thanks
The ER doc did 2 ECG's about 40 mins apart and the same results came out. I believe he did that to rule out possibility of error? Looking at the tape does it appear that I had a heart attack? Thanks,
doctor

Brief Answer:
No serious abnormalities on the ECG

Detailed Answer:
Different people have different constitutions, some are thin, some are normal and some are overweight. And they have different types of chest. So the heart axis can differ from individual to individial. In your case it seems to be normal.
The most exact answer will give the Echo.
And you will need holter monitoring if you have palpitations.

Hope I could answer to your question.
If you have further questions, don't hesitate to ask.

Have a good health

My last question is by looking at my ecg, does it appear I had a prior heart attack and damage to my heart? You mentioned there were slight r waves.

Brief Answer:
not seems like an old infarction in young patient

Detailed Answer:
Hi,

Yes, there are small R waves in lead V2 and V3, but it is only relative indicator for an old infarction. At first, the sensitivity of ECG in this case is not very high, and it depends also on your chest constitution and how they have placed the leads.
In this case the echo reveals the truth.
