Brief Answer:
there are several causes of coughing with blood

Detailed Answer:
Hello and welcome in HCM

According to the history (COPD & Atrial fibrillation ) the vascularisation in your airways is fragile and strong efforts may cause their rupture and as a consequence the blood in the secretions.
Another cause might be the hyper anticoagulation status of your blood (if you are taking anticoagulant ). INR values help answering this.If they are high than thia is the cause

Despite the above other lung diseases may cause coughing with blood and a chest X Ray is the first examination to do.Than if the blood continues than a bronchoscopy examination is needed. I am sure your doctor will advice you for the above.
