- Dilated food pile means there is an obstruction somewhere in the food pile. The causes can be stricture, cancer or Achalasia cardia associated with less motility due to diabetes.

If she was my patient, I would have order the following investigations :
Endoscopy, biopsy
CT scan
Barium study if necessary
CT scan

It shows Esophageal varices. These will be secondary to some liver or abdominal pathology like liver cirrhosis or so.
The Doctor will do further many tests including ultrasonography of the abdomen, all blood tests. The varices will be treated by banding or so.
The root-cause which caused this has to be found out and treated under the guidance of a Gastroenterologist.
