Question: I just received an elevated ALT result of 259 (I was not told the AST number although the doctor did mention that it was also elevated). I have been eating poorly and drinking more frequently than I should. I am approximately 15 pounds overweight and am a 38 year old female. By eliminating alcohol and increasing exercise, when could I expect a decrease in these levels on a best case scenario? My doctor said that I could go for another blood test after a week and I am wondering if this is a reasonable expectation.

I understand your concern.
In fact I am glad that you have decided to stop all these activities that are causing problems.
Rise in ALT and AST is probably due to alcohol intake.
However you should get following more investigations to rule out other causes and also to see condition of liver:
1. Ultrasound Abdomen
2. Complete LFT
3. Viral Markers.
4. Lipid profile
If you leave alcohol completely and do regular exercises I expect that your enzymes would come down to normal within 3 weeks.
You don't have any scope of consuming alcohol further.
If you have been eating poorly too then also I suggest you to take Thiamine supplement for a month.

See, your enzyme levels are increased because you consume alcohol, if you stop alcohol your enzymes would settle, but if you start taking alcohol again, they will again rise.
