Brief Answer:
Internal injury of knee likely.

Detailed Answer:
Hello,
Thanks for posting your query.
You must have hurt your internal structures in knee during marathon. It needs proper clinical examination and relevant investigations to diagnose which structure is hurt. The possibility of ligament, meniscus, capsule or synovial injury is there. These are best visualized on MRI (Magnetic Resonant Imaging). This is not due to a sprain.

You need to give rest to your knee. If it gives you relief you may resume sports after a gap of 2 or 3 weeks. A pair of knee supports can add to stability. But that again depends on your exact condition. If you don't get relief, proceed with MRI.
I hope this answers your query.
