What Does My ANA Test Report Indicate?

Question: I recently had a Anti Nuclear AB Reflux cascade and tested positive via Labcorp for Neonatal Lupus 100%. What does that mean in normal words? Thank youPlease help me interpret these results. I was tested because i have dry eyes. I'm using ductile plugs and Restasis.

Well, every lab has a different way of testing and giving out results, but overall it means the test has come out positive. Neonatal lupus doesnt fit in with your age, but mostly it coincides with Anti RO and Anti LA antibody which can cause the symptoms of dry eyes and dry mouth corresponding to a disease called Sjogrens Syndrome.

I would like to know a few more details before guiding you further from here:

1. Do you have any joint pains/fatigue/oral ulcers/hair fall?

2. How has your obstetric history been?

3. Did they check for other labs as well...like ESR, Urine routine, Complete blood count, kidney function tests?

4. If you could attach the ANA report here , it would be helpful. Wanted to see the exact titres, pattern and further ENA if they have done

Thank you for your response.
1. I have some fatigue, but I am raising a toddler who wakes at night 1-2 times per week still. I have some back pain but i think it is related to posture. It feels more muscle related. I don't have any other joint pains, oral ulcers or hair loss.
2. I was delivered healthy, full term vaginally and I delivered 1 healthy son full term vaginally. No miscarrages. Concieved on the 2nd month trying.
3. I have attached my CBC
4. Lab did not give exact titers. My doctor has requested this and I am waiting on that information from the lab.and any other information from the blood work that I currently have

Sorry for the late reply.. Had a long duty which just got done

I have gone through your results and the reports seem quite ok. So one thing is reassuring is there is no internal organ involvment.

As for ANA positive, we do need to see how strongly it is positive as 1: 40 is also labelled as positive and 1: 320 dilution. Anything above 1: 160 is considered significant.

Secondly, After ANA we need to check for the ENA profile. It is a subset of ANA which checked around 15-20 antibodies and the manifestations depend on the type of antibody you have.

You may have syndromes of Sjogrens Syndrome but look mild so far. ENA will help determine the same. As of now, you dont need to be too worried. I would recommend you let me know the ENA result and consider starting on Hydroxychloroquine. It is a relatively safe drug which halts the further disease progression

I really appreciate the information! It makes sense to me. I did notice that my original attachments (the ANA full results and the results of the SSA and tge SSB) did not go through with my 1st paid question so I wanted to include them now. I tried contacting customer service about how they did not go through, but haven't been successful yet because the email keeps bouncing back. I did get the attachments, but I have been trying to figure out which one they have said positive in your case. The attachments show all the antibodies and the percentage positivty in different diseases but not the actual one positive. It is more of the information one. I could not find the ANA report either.

Sorry. Maybe I am doing this wrong. This is all very confusing to me. Attached is the most recent information that I recieved. It states anti DNA (DS) Ab/Qn is 30 IU/ mL. Does that give you a clearer picture?

Well, I managed to get this result and it seems positive. They havnt mentioned the cut off, but the usual is 12 and since it is highlighted in red , I am assuming it is positive.

Well the significane of DsDNa is monitoring of the kidneys. We need to look at your urine routine as well along with C3 and C4.

But all in all, I think it is a start of the disease and we need to see how it evolves in your case. Some of them remain mild, some evolve differently. Antibodies checked are used to indicate which organ may get involved and how vigilant of monitoring we need.

But, we treat the symptoms and not the antibodies. You can have a few antibody positive but the disease may remain mild, so you may not need heavy treatment. That we will know by looking at your remaining reports and observing you over time.

When you say "the disease" which disease are yoy referring to? Sjogren's syndrome or Lupus?

Good evening

Well, for now you dont have enough symptoms to fit into SLE. Only thing which points towards SLE is Anti Ds DNA positivity, which alone may not hold much significance.

The symptoms of dry eyes along with ANA goes more in favor of Sjogrens Syndrome. Never the less, both belong to the same family but in SLE we just need to monitor organs more vigorously than Sjogrens.

Hydroxychloroquine works well in both the conditions.

If you ask my diagnosis I would label you as Undifferentiated Connective tissue disease...That is you have a few symptoms but not fitting well into a disease but Antibodies are coming positive. There is a chance it may evolve further either into SLE or Sjogrens or there is a chance it remains this way for next few years. That we would be sure with Detailed ENA profile and observing you for evolution of symptoms. Hydroxychloroquine will help slow down the progession of the disease
