Question: I have had a CT abdomen/Pelvis with contrast and the findings are: there is asymmetric soft tissue in the rectum noted posteriorly extending over an area measuring approximately 3.6 x2.1 x 3,5 cm. this is suspicious for possible mucosal neoplasm. there is no evidence of mesorectal lymphadenopathy or invasion into other adjacent structures. endoscopic correlation is recommended......my concern is if cancerous and cut into it wll be in my bloodstream and metastize...the report did add that there is no evidence of regional or distant lymphadenopathy or metastatic disease......your opinon pls

Brief Answer:
You require an urgent colonoscopy and biopsy to confirm diagnosis

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Your CT scan abdomen and pelvis shows that there is irregular wall thickening involving the rectum. Such asymmetric wall thickening in the rectum must be biopsied at the earliest and treated as per pathological findings.

2. A neoplasm is another word for a tumor. While your condition is likely to be a tumor in its early stages, there is an early need of doing a colonoscopy and then proceeding with a biopsy to confirm the diagnosis.

3. As there is no involvement of adjacent structures and no lymph nodes are present, even if it is a cancer, the chances of successful treatment is higher.

4. In my experience while working with patients having cancer of colon and rectum, even without any lymph nodes or adjacent spread, there is a 50 percent chance of metastasis in patients receiving recommended treatment for colorectal cancer This means that one out of two patients having cancer of rectum and without any regional or distant metastasis will end up with metastasis subsequently even when provided with adequate treatment.

5. However, please do not worry and get your colonoscopy and biopsy done at the earliest so that the condition is ruled out or confirmed.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
