Question: My uncle got BMV surgery in 2010 as his heart valve was narrow.He was alright then. From few days , he got pain in Stomach. He was diagnosed with Gall bladder stone and decided to go for a surgery for Gall bladder stone. He used to take otrivin but stopped it as he has to go for surgery. He was not able to go for surgery as he got some fungus infection . He again continued otrivin, On 2nd April , he was talking with my brother and suddenly he fell down as he is sleeping. My brother, then took him to a nearby hospital in XXXXXXX as it was night and it was not possible to take outside. The Doctor diagnosed him and found that he would have got brain stroke either due to clotting or hemorrhage. They did CT Scan and found that it not hemorrhage or clotting. They treated him for clotting as clotting can be viewed in Scan only after 4 hours.They have given skt thinner.. The patient was talking and was responding Well.Then they took him to XXXXXXX and the doctor told the same thing .They again took him to New XXXXXXX Vedanta, the doctor are diagnosing him and told that there is an active clot in heart and he even got hemorrhage in brain, The doctor are giving light thinner as its mandatory due to heart but it can cause bleeding in Brain.My uncle was talking a bit and recognising all till yesterday but he is just sleeping today. Please find his attached report and bring us out of this critical situation.

Hi,

Thank you for posting your query.

I have noted your uncle's reports and clinical details. He has suffered from a large brain stroke, affecting the left side of brain, due to reduced blood supply there (embolic stroke, due to clot from the heart). The left side of brain controls speech and movements of right arm and leg, so they have got affected. Any improvement would be slow and would take several months.

The medicines to dissolve the clot, such as warfarin or acitrom should be continued with caution, as there is small amount of bleeding too. He is on correct treatment.

Speech therapy and physiotherapy should be continued.

There is no urgent treatment available now.

Thank you for getting back.

Unfortunately, there is no urgent treatment available for brain stroke at this stage. Any urgent treatment is useful only within the first four and half hours of paralysis, which is over long time back.

At this stage, there is no medicine available to make the better better/normal. The blood thinners are needed for the clot in the b=heart, which needs to be decided by the cardiologist.

For the neuro problem, treatment is with speech therapy and physiotherapy.
