Question: Hi Doctor,
My sister getting fever and pain after uterus and ovary removal surgery.post surgery doctor suggested to take ct scan k.u.b(plain &contrast).In that report we observed like below
LEFT RENOMEGALY contrast excreation delayed.
mild dilation of pelvi calycial system noted
kinking noted in pelvi ureteric junction

doctors saying that this is about kidney infection medicine is enough to take like that but from fast 5 days she is getting same pain.kindly reply to this.

1. The CT scan report shows that the left kidney is enlarged in size and is showing delayed excretion of contrast which means that there is a possible infection which has appeared after the surgery to remove uterus and ovary.

2. Such an infection can originate in the urinary bladder and is probably affecting the left kidney.

3. It is important to send her urine and blood sample for culture and sensitivity to know which bacteria is causing the infection. A strong antibiotic can then be prescribed which will treat the infection and she should feel better within 3 days of starting treatment. The pain can be due to the infection and because she recently had the surgery. Pain relief medicines are to be given after examining the patient if required.

Kindly refer doctor in XXXXXXX ,from past 5 days current doctor Kotareddy nursing home kandukuru used lot of antibiotics ,but no use. daily she is getting fever and pain.
Tomorrow only I will get updated reports .we are worrying about this situation.
We planned to go XXXXXXX hospital tomorrow.

1. We understand that Kandukur is a small town and therefore you would like to take her to a higher center for better treatment.

2. It is important to take her to a place where there are multiple specialists. If you are close to XXXXXXX hospitals and feel that she will get better care then you can first consult a urologist or nephrologist doctor and then decide on shifting the patient.

3. XXXXXXX is a big city and there are many good specialists in urology. We are not liked to any particular hospital but suggest that Madras Medical College specialists can handle such cases. CMC Vellore is an alternative hospital where you can get good treatment.

In conclusion, please have a discussion with treating doctor and get reports showing which antibiotics will work best for the kidney infection even if you decide to get her treated in another hospital. This will help to start required treatment quickly.
