Question: Got results of CT of abdomen/pelvis with contrast. Can you translate please?

1. Your spleen is enlarged. This is an organ in the left side of upper abdomen. It shows a 1.2 cm complicated cyst within and is likely benign.

2. A 1.3 cm harmless cyst is there in upper pole of left kidney. This is not a problem.

3. Both ovaries show cysts. Right ovary shows many cysts largest measuring 1.7 cms. Left ovary shows a 1.3 cms cyst. It appears that the cysts have thick fluid and therefore complex in nature.

4. The uterus shows an area which is suspicious for a fibroid which is a benign tumor of uterus and is commonly seen in females. There is slight fluid in the area between the uterus and rectum. This can be normal or abnormal depending on your menstrual cycle.

In conclusion, it is important to know if you have any condition which is causing and enlarged spleen. The uterus and ovaries require a follow up ultrasound scan in 6 weeks to assess the cysts in ovary.

1. The enlarged spleen can be from an infection or fever. The cyst is not causing enlargement of spleen and seem to be a harmless entity.

2. The thick fluid in ovary cysts indicate that they are not simple fluid containing cysts and probably point to chocolate cysts or hemorrhagic cysts. Their size is small and not a serious concern.

3. Fluid in the space between uterus and rectum is usually found around ovulation which usually occurs 14 days before the onset of periods. I guess the fluid amount is small in your case since not much has been written about it. If you experience white discharge from vagina then the fluid can indicate pelvic inflammatory disease.

4. The spleen enlargement is not connected to any other problem. The cyst in spleen, kidney and ovary are not relative.

5. The ovary cysts and fibroid uterus can be due to hormonal imbalances.

1. Spleen: I have no fever, but have been feeling pain under left rib, left flank and left back. I assume this is from enlargement? So, to be clear, nothing on this report would cause the enlargement...these are not infections?

2. Would the CBC I took today help me understand if I have an infection, and if so, possibly where?

3. What are the best and worst case scenarios? Mono? Leukemia? Passing infection?

4. The "hormonal imbalances" you mention...could that be from where I am in my monthly cycle? Or perhaps I am nearing menopause? Would that explain anything?

5. I feel like you're saying the only potential concern here is my spleen and the reason for its enlargement. Do you agree?

1. The pain under left ribs and in left flank is probably from the enlarged spleen. I am not sure if the complex cyst in spleen has anything to do with an infection and it is sometimes difficult to make such a comment based on CT scan.

2. CBC will show increased white blood cell counts if you have any infection and the ESR will be increased.

3. The best case scenario is a splenic enlargement from a passing mild infection or a normal spleen in case you are over weight and. The worst case scenario is to be judged with other clinical features like fever, weight loss, tiredness and significantly increase white blood cells which can indicate leukemia. However a leukemia will usually cause a spleen larger than 15 cms.

4. The hormonal imbalances can be due to menopause which is close or consumption of contraceptive pills and mental stress.

5. The spleen enlargement is a concern if you have been feeling pain below left ribs and in left flank. This is something which requires detailed clinical evaluation.

Sorry if it seems like I am asking the same question in ten different ways, just trying to narrow down the possible causes.

1. I definitely have/have had pain below left ribs/left flank/left back. As well as fatigue. Does that make this more of a concern? What kind of detailed clinical evaluation should I be pursuing?

2. I don't think it is a passing infection because it's been feeling this way for months and had been looking into other causes: heart/chiro/reflux.

3. I have recently gained a lot of weight, which I guess is good since I'm not losing weight, but that could be why it seems enlarged? But then would I still have pain?Weight gain mostly due to discomfort when working out, so barely exercising anymore.

4. You mentioned the spleen being more than 15cm with Leukemia. Was that reference to mine being 15.7cm? Or is 15cm used as a barometer for possible leukemia? It wasn't clear to me....

1. Your questions are important and I would have asked the same way if I were in your place.

2. Clinical evaluation should include a detailed blood work up including a peripheral blood smear. This is to be followed by a physician consultation.

3. If you feel this way for months then a mild low grade chronic infection might be the cause. Leukemia is unlikely if the symptoms are there for longer than 6 months.

4. If a person is over weight from before then they can have a larger than normal spleen depending on build. Weight gain due to not exercising will not cause enlargement of spleen. The pain from an enlarged spleen is due to stretching of spleen capsule and pressure on the stomach.

5. I take upper limit of spleen measurement up to 13 cms as normal. If it is 15.7 cms then there is mild enlargement. In leukemia the spleen is huge and usually crosses 20 cms. There is no 15 cms demarcation regarding spleen measurement and leukemia, my reply was based on experience and I have worked in a cancer institute for few years.

Thank you...that was very helpful.

1. My doctor has only ordered an ultrasound of the pelvic area in 6 weeks because of the ovarian cysts. I only had blood drawn today at my request.

2. Should I be concerned with the attention paid to my results, as they didn't even mention the spleen enlargement to me when they called with the results? Only the ovarian cysts. Especially since I went to see him for pain on my left side?

3. Should I get another opinion? See a specialist?

4. What would be an example of a "mild low grade chronic infection"?

5. What do you think is going on? Should I be concerned, in general?

6. Looking at the whole picture, is it abnormal to have cysts in so many different places (spleen/kidney/uterus/ovaries)? Or would this be what you might find in most people if they had a CT scan?

1. Ultrasound scan of the pelvis after 6 weeks will show if the cysts are the same size or have increased in size. This is a routine procedure when cysts are seen in the ovary. It is good that you had a blood test done from your side.

2. If you had pain in the left upper abdomen below the ribs and your spleen measures 15.7 cms then it is an area which requires a detailed examination.

3. You can have a discussion with an internal medicine specialist regarding enlargement of your spleen and the pain you have been having.

4. An example of a chronic low grade infection can be mononucleosis or Lyme disease. Please note that this is only an example and without clinical examination it is not correct to make a probable diagnosis.

5. An enlarged spleen is not always a serious problem but since you are having the pain in the area therefore it is always better to have a complete over view of your condition.

I would say that the cysts in the ovaries and fibroid in uterus are common in women of your age. A single cyst in the kidney is common in many people. The cyst in spleen is less commonly seen.
