What Does My CT Scan Test Result Indicate?
Question: CT Scan shows extensive opacification within the ethmoid, sphenoid, and frontal sinuses.
I had a CT Scan of the paranasal sinuses. There is extensive lobulated mucosal thickening noted in both maxillary sinuses widening and occupying both of the ostiomeatal complexes. There is extensive opacification noted within the ethmoid, sphenoid as well as the frontal sinuses. There is obliteration of air within the sinuses due to the extensive mucosal thickening.
There are no fluid levels noted. There is no significant deviation of the nasal septum. The anterior nares and posterior nares appeared normal. The post nasal space also within normal limits.
IMPRESSION: Extensive opacification of the sinuses from chronic sinusitis/polyposis
What are your thoughts on this?

1. The report says that all the paranasal sinuses are blocked, which is impairing the ventilation and normal drainage of the sinuses. It indicates an allergic aetiology.
2. Even after completion of 2 courses of oral antibiotics and a short course of oral corticosteroid along with intranasal sprays, the sinuses still remain blocked. Hence, the extensive blockage of the sinuses impairs the effective reach of medicines into the sinus cavities. Unless the block is not relieved, your symptoms will not subside completely. This is a strong indication for surgery.
3. My honest suggestion to you is to get the nasal surgery done. The surgery will be done endoscopically without any scar from the outside. After surgery, you should continue using Nasonex and saline nasal spray for a longer period.

I did manage to get allergy testing done when I saw an ENT Specialist.
I'm allergic to house dust mites, grasses, and pacific trees. My Histamine test was 6 mm positive. Phenol and saline tests were negative. I work at a lumber yard/ building supplies store and spend some time cutting MDF baseboards, PVC plastic sheets, plywood, and other lumber. The saws I use dont have any dust extraction on them so I breath in quite a bit. Haven't had issues with itching or constant runny nose. I can usually breath through my nose until bedtime.
Im currently prescribed Dexedrine Spansules and Wellbutrin XL for treating my ADHD and Depression. Also on clonidine .2mg for sleep. I was on quetiapine 25mg for sleep but that was changed 3 days before my allergy testing.

The thickened mucosa blocks the ostium of the paranasal sinuses. When the assault is uncontrolled, the edematous mucosa lining the sinuses becomes polypoidal.
In your case, unless the surgical intervention is not done, the symptoms will never get better. Following surgery, the medicines will be able to reach every corner of the paranasal sinuses effectively, there by, counteracting the allergic reactions.
Also, the preventive measures if followed religiously, can bring down the quantity of the medicines used. Usage of a good mask to prevent the fine dust is always recommended.
