PSA levels have been up and down for some time Scan also revealed enlarged prostate so does this mean he could still have a tumour... would the tumour not be visible on the scan?

Well, most tumors are seen on scan, but in some cases it may be small and this MRI finding may indicate something like this. But as I said, this is probably just benign prostatic change. It is necessary to have PSA level checked on regular interval and ultrasound follow ups and if there will be some further enlargement, then biopsy should be done.
