What Does My MRI Scan Report Indicate?

Question: I am a 64 yo male 6'4' 323 lbs. For the last 2 months I have had occasional balance issues. These occur after I sit for a extended period of time at work mostly. I have found the issue occurs after sitting and leaning forward working on the PC. When I stand my Face reddens, ears feel full, I develop a headache and my legs become heavy. This will last for a few minutes or until I sit. My B/P elevates 150/100 standing and pulse stays around 60. Sitting B/P 130/ 80 P 60. MRI results have not changed in 5 years (which shows diffuse white matter disease). This does not occur when lying or sitting in a reclined position. I'm a RN so I'm starting to think it is Cardiac or Vascular. My ejection fracture is 38 . What is your thoughts

Detailed Answer:
Hello!

Welcome on HCM!

Regarding your concern, I would explain that your symptoms are not related to any cardiac disorder.

The headache and ear fullness, while bending forwards is typical of a sinus infection or congestion. For this reason, I would recommend performing a sinus discharge bacterial culture. If a bacterial or fungal infection is excluded, I would recommend taking an antiallergic agent, because an allergic reaction can cause thickened sinuses from nasal congestion. The nasal decongestionant would also help.

A vascular problem in the brain can not be excluded either, as your MRI shows diffuse white matter disease, which is an indicator of small vessel disease.

I would recommend taking daily Aspirin for this reason.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

Wishing all the best,

Dr. Iliri
