Brief Answer:
Need at least an MRI

Detailed Answer:
I would look TO DO an MRI as a more precise way of diagnosing the cause of the headaches which sound to be some form of HYDROCEPHALUS. In a young woman of your age my guess is that you have something that USED TO BE CALLED pseudotumor cerebri.

We now have another name for the condition called IDIOPATHIC INTRACRANIAL HYPERTENSION. Things highly associated with this condition include obesity, overuse of Vitamin A or their analogues such as retinene or retinoic acid.

There could also be organic types of problems for this type of condition which would not otherwise be picked up with a CT scan hence, the MRI which is much more precise.

Although mannitol is considered a standard drug it has an aggressive side effect profile. I find that drugs such as topiramate and zonisamide work just as well at lowering pressure without some of the undesirable side effects...although topiramate is known for some funny tingling sensations in the head and neck as well as extremities called paresthesias and if this happens we have to stop the drug.

Brief Answer:
Your symptoms merit immediate examination by an ER physician or neurologist

Detailed Answer:
You may be developing a chronic case of meningitis with those types of symptoms though I am not necessarily working off of the imaging study you mention of the CT scan because I don't detect any suspicion by the radiologist that such is present on their scan. However, MRI are much better to tell if there is a meningeal condition such as an infectious meningitis which could also be contagious. Therefore, I believe you need to be seen for that and if your history and examination are consistent with that diagnosis you will need a LUMBAR PUNCTURE and to receive IV antibiotics immediately until culture results return at which point treatment can be stopped if no infection is found.

Bottom line: I would get thee to an ER tonight and hopefully, they will admit you at least for observation of the fever and an LP if appropriate as well as a good MRI of the brain (possibly with Gadolinium contrast although you also should be cleared renally first since the contrast can be really rough on the poor kidneys.
