1. What is the symptomatic improvement after H.Pylori treatment: I still have pain in middle of chest. However do not have symptoms such as heartburn, reflux. initially also, I only had pain and discomfort in the middle of chest and back and not much symptoms of reflux. However it was only in the endoscopy that it came positive with mild reflux esopgatitis.
2. Back ache intensity , severity: The pain sometimes starts at the back and radiate to the front and sometimes in the lower chest below sternum and radiates to back. I have noticed that I feel some discomfort while sitting. The intensity is medium to severe for 15-20 mins but then it then complete resolves after that.
3. Associated nausea , vomiting , fever , loss of appetite: None, but i have lost 2 kgs in last one month may be due to diet control
4. Bowel movements: Normal
5. Consuming alcohol , tobacco in any form: No, drink alcohol last time about one week back, otherwise very occasionally. Can amalyse be increased due to this, test was done on Monday (3 days after)?
6. Stress level and sleep pattern: Stress normal; sleep is fine, pain never awakes me from my sleep.
I also have an query: Due you think I should get my amylase test again since my ultrasound came normal for pancreas and gall bladder. What could be other test that I can go for to rule out pancreatitis. Currently i am having PPI and Ultizyme.

- Alcohol is a major factor affecting serum amylase levels , so do not consume
alcohol in any form for further 7 days and then repeat the sample .
- Other test is CT abdomen to rule out pancreatitis , but I do not think you require
it at present time .
- Continue your current medications , diet management with additional advise to
do regular walking , exercises , deep breathing , YOGA for better results .

I visited a doctor today for my condition who also asked to get chest x-ray done which was normal. He said that it could be a case of esophageal spasm. What are you opinion on this?
He has prescribed me Zantac and Dom DT (Domperidone) for 2 weeks and said that he would get manometry done in case the condition does not resolve in the time period. He also said that amylase is normal and we should not worry about it.
Sir what is your opinion on the same. I hope my condition is resolved as the pain is really becoming irritating now.

1. As per the presentation , esophageal spasm was not suspected , but still okay
x-ray chest rules out many things .
2. As in my previous narrations , amylase was not that level to be worried for
severe pancreatitis .
3. I strongly support that pain is from the ulcer part , must be settled with proper
medications , life style modifications , diet management .
According to the progress you may not require manometry also .
* Suggestions for better recovery
- Continue current medications .
- Soft , light diet at intervals .
- Avoid oily , spicy , non veg stuffs , hot beverages .
- Drink plenty of liquids .
- Avoid prolong fasting or over eating .
- No alcohol intake in any form .
- Have regular sound sleep of 8 hrs.
- Regular walking in fresh air , exercises , deep breathing , YOGA help a lot .
