Question: PERIPHERAL BLOOD SMEAR : RBC's show anisopoikilocytosis and are microcytic 0
hypochromic with few few elliptocytes,tear drop cells
,target cells with occasional acanthocytes and fragmented
cells.
WBC's are normal in total number and distribution.
Platelets are adequate
No parasites are seen

QUESTION : Why has doctor asked for the above mentioned tests i.e FERRITIN, OSMOTIC, FRAGILITY and SICKLING

If he has no any symptoms then no tests are required.

I have gone through your question. I can understand your concern.

He has slight microcytic hypochromic blood picture but his hemoglobin is normal. He may have slight iron deficiency.

Ferritine is a iron storage form. Ferritine level is tested to check iron deficiency.
Osmotic fragility test is used to diagnose thalassemia and hereditary spherocytosis.
Sickling test is used to diagnose sickle cell anemia.

But his hemoglobin is 12.9 so there is no need of all these testing if he has no any symptoms. If he has any symptoms then please tell me about that.

Thanks for your reply doctor XXXXXXX the lab doctor said she wanted to do these tests. I also could not understand that given all the test are normal why she needed to do all these sensitive tests. I and my wife are really worried after reading about osmotic fragility. Also none of us has and blood related genetic disease like (thalassemia). Do we need to take any second opinion or wait for the report.

One additional thing, the reason we went for above mentioned test because he was having fever every 10 days for day or 2. We stay in XXXXXXX and weather here is very changing even in a single day. Can regular fever also lead to low levels or iron in the body.One small additional question, the lab doctor also asked me did my kid has any case history which could lead him to be anemic. but given your statement, he is not anemic.

Fever can be due to some infection. He may have some bacterial or viral infection. But it does not have any relation with above mentioned test.

His hemoglobin is very good for his age. According to me these all testing are not required. No second opinion is required for above mentioned condition. Consult your doctor and take treatment for his fever.

Slight iron deficiency may be due to nutritional or there may not be any iron deficiency. So its not a matter of concern.

If both of you don't have any disorder then there is very rare chance that your child has this genetic disease.
Just be relaxed. If needed repeat the CBC.

Hi Doctor, your answer was so much relieving. Just one last thing for today. the lab doctor mentioned the size of the cell was smaller, what does that mean and how much smaller cells can be due to repetitive fever etc and anything else if you wana tell.

Smaller RBC is commonly seen in iron deficiency.

In your report MCV indicate the size of RBC. Its normal value is 76 to 94 fl. If it is less than 76 then it is called as microcytic(smaller RBC).

Fever as such as no effect on RBC size. Slight variation from range is also common. Nothing to worry about that. Give him iron rich food like green leafy vegetables, jaggary, dry fruits etc. Nothing much is required for it.
