What Does My Blood Test Report Indicate?

Question: Hello Doctor, My Daughter is 10 months old . She has been getting fever ever month due to infection .
Last month WBC count was 11400 cells/ Cu mm and this month its 20400 cell /Cu mm .
In serology test (widal test test slide method ) SALMONELLA TY PHI "O" is 1 in 160 Dilution.
In serology C reactive protein Qualitative is positive, semi quantitative is TITRE TO 96 mg/L ( DIL 1:16) .

Why is my daughter getting this infection on monthly basis?
Local Doctor prescribed Antibiotics last time and now , Is is safe to use the anti biotics ? will it not affect the immunity of the kid ? How to prevent this infection ?

1. In the past 6 months how many episodes of fever occurred?
2. Each episode lasted for how many days?
3. During each fever episode - when the fever is low, how is the activity of the kid?
4. Every time what are the symptoms associated with fever - like - cough/ cold/ breathing problem / crying while passing urine/ blood in the stools/ seizures etc.
5. What was the birth weight and what is the current weight?
6. How is the attaining of developmental milestones?
7. Is the stool oily and floats in the pan?
8. Is there any color change in the hair or skin?
9. Is there any history of recurrent serious infections?

Please answer all the questions and get back to me.

27/8/14 pentavac PFS vaccine shot
29/8/14 3 days (paracetomal and injections )
7/02/15 MVAC OPV shot for measels
9/02/15 3 days (paracetmol and other oral drops for cold )
14/2/15 5 days (taken injections)
9/3/15 3 days (paracetmol and other oral drops for throat infection)

18/3/15 now suggested to take injections anti biotics

3. During each fever episode - when the fever is low, how is the activity of the kid?
if temp is low kid is super active and playing .

4. Every time what are the symptoms associated with fever - like - cough/ cold/ breathing problem / crying while passing urine/ blood in the stools/ seizures etc.

Crying when at high temparatures .
Body reflexes and sighing when at high temp at night .

5. What was the birth weight and what is the current weight?
birth 4.35 kgs (born to a gestational diabetic mom) now 9.25 kgs

6. How is the attaining of developmental milestones?
Good developmental milestones.
Crawling, toothing , getting up

7. Is the stool oily and floats in the pan?
not oily and not floats

8. Is there any color change in the hair or skin?
During fever under arms become little darker.
9. Is there any history of recurrent serious infections?
as per the reports the only recurrent infection is

wbc count at brith 12,900 in feb 10900 , now 20400 cell/ cu mm

TITRE TO at birth & in feb 24 mg/L (1:4), now 96 mg/L (1 :16 )

This is recurrent viral illness - DO NOT WORRY

Detailed Answer:
Hi...I am very sure that this is viral illness and antibiotics and antibiotic injections have been used unnecessarily.

Issues in your kid -

1. The fever pattern and frequency you are describing is quite normal for many kids of this age group.

2. When the kid is active in between 2 fever episodes - you need not worry.

3. Widal test should be done only after 7 days of continuous fever for best results. There are 2 methods of doing this - slide method and tube method. In slide method the result will be fast and very inaccurate. It should not be used. The tube method is the best and will take 24-36 hours to report. It is good that you mentioned that the kid's Widal test was done in slide method - do not trust it.

I will suggest you what a viral illness is and will also suggest what to do and what not to do -

Fever of few days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

Another most commonly misused drug is Mefenamic acid - Meftal-P contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Meftal-P.

Hi...

1. High WBC counts can occur in any type of fever. if the kid is active in between two fever episodes on same day - no need to worry about high counts. This is not harmful.

2. Immunity cannot be improved by food. The only way to improve immunity is by vaccinating the kids with all available vaccines.

3. Widal test of slide method is a very poor test - Do not worry about its titers. This is not typhoid fever.
